目的探讨血管紧张素Ⅱ2型受体(AT2R)与血压水平及相关炎性因子的关系。方法采用横断面研究方法,从体检人群中随机调查138例为研究对象,根据美国预防、检测、评估与治疗高血压全国联合委员会第7次报告(JNC7)将入选对象分为3组,正常血压组31例(≤120/80mmHg)、高血压前期组39例(120~139/80~89mmHg)、高血压组68例(≥140/90mmHg);用酶联免疫吸附技术(ELISA)检测血浆AT2R水平,用放射免疫法检测肿瘤坏死因子α(TNF-α)、内皮素1(ET-1)及血管紧张素Ⅱ(AngⅡ)的水平,用自动生化检测仪检测血脂、血糖、肾功能。结果血浆AT2R在正常血压组、高血压前期组及高血压组间差异有统计学意义(P<0.01),且与收缩压(r=0.881)、舒张压(r=0.752)、平均动脉压(r=0.847)及脉压水平(r=0.587)呈正相关(P均<0.01);2)血浆AT2R与TNF-α(r=0.804)、ET-1(r=0.769)及AngⅡ(r=0.806)呈正相关(P均<0.01);3)多元线性逐步回归分析显示,年龄、AngⅡ及AT2R是平均动脉压的独立影响因素;AngⅡ及AT2R是收缩压的独立影响因素;ET-1及AT2R是舒张压的独立影响因素。结论血浆AT2R水平与血压水平及炎性因子水平呈正相关,是平均动脉压、收缩压及舒张压的独立危险因素。
Objective To investigate the relationship between angiotensin Ⅱ type 2 receptor (AT2R) and blood pressure and related inflammatory factors. Methods According to the cross-sectional study, 138 subjects were randomly selected from the physical examination population. According to the 7th National Joint Committee on Prevention, Detection, Evaluation and Treatment of Hypertension (JNC7), the subjects were divided into three groups: normotensive (N = 120 / 80mmHg), 39 cases (120 ~ 139/80 ~ 89mmHg) in hypertension group and 68 cases (≥140 / 90mmHg) in hypertension group. The levels of AT2R in plasma were detected by enzyme linked immunosorbent assay (ELISA) The levels of tumor necrosis factor α (TNF-α), endothelin-1 (ET-1) and angiotensin Ⅱ (AngⅡ) were detected by radioimmunoassay. Blood lipid, blood glucose and renal function were detected by automatic biochemical analyzer. Results Plasma AT2R levels were significantly different between normotensive group, prehypertensive group and hypertensive group (P <0.01), and were significantly correlated with systolic pressure (r = 0.881), diastolic pressure (r = 0.752), mean arterial pressure (r = 0.847), ET-1 (r = 0.769) and AngⅡ (r = 0.806, r = 0.847 respectively) and pulse pressure (r = 0.587) (P <0.01) .3) Multiple linear stepwise regression analysis showed that age, AngⅡ and AT2R were independent influencing factors of mean arterial pressure. AngⅡ and AT2R were independent influencing factors of systolic blood pressure. ET-1 and AT2R were Independent factors of diastolic pressure. Conclusion Plasma AT2R level is positively correlated with blood pressure and inflammatory cytokines, and is an independent risk factor of mean arterial pressure, systolic blood pressure and diastolic blood pressure.
