目的:探索7.5%NaCl治疗模拟高原缺氧复合失血性休克山羊的效果及其可能机理。方法:雄性山羊14只,随机分为高渗氯化钠液治疗组(HS,n=8)和生理盐水治疗组(NS,n=6)。在模拟海拔4000m低压舱内停留24h后,经股动脉放血,使动脉平均压(MAP)降至(5.50±0.32)kPa,维持60min后,按4ml/kgbw剂量,由静脉分别输入7.5%NaCl液或0.9%NaCl液。输入NaCl液30min后,回输血液。结果:7.5%NaCl液可明显提高模拟高原条件下失血性休克山羊的MAP、左室收缩压(LVSP)、左室压最大上升速率(LV+dp/dtmax)与左室张力指数(LVTI);促进血液pH及血气参数逐渐恢复至接近对照期值;明显延长存活时间和提高存活率。结论:7.5%NaCl明显改善实验动物的心泵功能,改善血气与酸碱参数,具有良好的复苏效果,在高原地区失血性休克的现场救护提供了有益的启示。
Objective: To explore the effect and possible mechanism of 7.5% NaCl in simulating plateau hypoxia combined hemorrhagic shock goat. Methods: Fourteen male goats were randomly divided into three groups: hypertonic sodium chloride (HS, n = 8) and NS (n = 6). In the simulated altitude of 4000m low pressure cabin stay 24h, the femoral artery bleeding, the average arterial pressure (MAP) decreased to (5.50 ± 0.32) kPa, maintained for 60min, according to 4ml / kgbw dose from the vein input 7.5% NaCl solution or 0.9% NaCl solution. Enter NaCl solution 30min, the blood was transfused. Results: MAP, LVSP, LV + dp / dtmax and LVTI in hemorrhagic shock goats at simulated altitude were significantly increased by 7.5% NaCl solution. Promote blood pH and blood gas parameters gradually restored to the value of the control period; significantly prolong survival and improve survival. CONCLUSION: 7.5% NaCl can significantly improve cardiac pump function, improve blood gas and acid-base parameters, have a good resuscitation effect, and provide useful enlightenment for on-the-spot rescue of hemorrhagic shock in plateau.
