目的探讨肺动脉高压(PAH)对结缔组织病(CTD)患者左心室功能的影响。方法选取结缔组织病患者34例,合并肺动脉高压者(CTD-PAH组)18例,未合并肺动脉高压者(CTD组)16例。选取正常对照组18例。采集左室侧壁(left ventriculer lateral wall,LVLW)二尖瓣环位点组织多普勒频谱(tissue Doppler imaging,TDI)和二尖瓣、主动脉瓣口血流脉冲多普勒频谱(pulse wave Doppler imaging,PW),测量左室等容舒张期(IRT)、等容收缩期(ICT)、射血期(ET)并计算左心室Tei指数(LVLWTei,PWTei)。结果 CTD-PAH组与CTD组、正常对照组比较,CTD-PAH组左心室Tei延长(P<0.05);左心室LVLWIRT、PWIRT延长(P<0.05);LVLFET、PWET缩短(P<0.05);CTD组与正常对照组间比较,差异均无显著性。结论肺动脉高压明显损伤结缔组织病患者左心室整体功能。
Objective To investigate the effect of pulmonary hypertension (PAH) on left ventricular function in patients with connective tissue disease (CTD). Methods Thirty-four patients with connective tissue disease, 18 patients with pulmonary hypertension (CTD-PAH) and 16 patients without pulmonary hypertension (CTD) were selected. Select the normal control group of 18 cases. Tissue Doppler imaging (TDI) and mitral valve were recorded at left ventricular wall of left ventricular wall (LVLW). Pulsed Doppler echocardiography (Doppler imaging, PW). The left ventricular isovolumic tension (IRT), isovolumic systole (ICT) and ejection duration (ET) were measured and the left ventricular Tei index (LVLWTei, PWTei) was calculated. Results Compared with CTD group and normal control group, the left ventricular Tei in CTD-PAH group was prolonged (P <0.05); the left ventricular LVLWIRT and PWIRT were prolonged (P <0.05); LVLFET and PWET were shortened (P <0.05); CTD group and the normal control group, the difference was not significant. Conclusions Pulmonary hypertension can obviously damage the left ventricular function in patients with connective tissue disease.
