目的:研究药物治疗对妊娠晚期细菌性阴道病(BV)不良妊娠结局的预防效果。方法:对2006年1月~2007年1月在我院产检的孕28~36周的孕妇进行BV筛查,将筛查出符合收治标准的150例BV患者随机分为妇洁洗液组、甲硝唑栓组、乳杆菌活菌胶囊组,每组各50例。甲硝唑栓组,乳杆菌活菌胶囊组均为1枚药物阴道给药,妇洁洗液组为妇洁洗液20ml化水清洗外阴,均连用10天停药,1周后复查。结果:妇洁洗液组50例,无1例治愈;甲硝唑栓组50例,45例治愈,2例显效,总有效率达94%;乳杆菌活菌胶囊组50例,40例治愈,4例显效,总有效率88%。甲硝唑栓组与乳杆菌活菌胶囊组在治疗效果上比较,两组间无显著性差异(P>0.05)。早产、胎膜早破、未足月胎膜早破、低体重儿、新生儿黄疸的发生,甲硝唑栓组和乳杆菌活菌胶囊组均明显低于妇洁洗液组,差异有显著性(P<0.05)。结论:甲硝唑栓和乳杆菌活菌胶囊对妊娠晚期合并BV的不良妊娠结局有明显的预防效果,是治疗妊娠晚期BV的安全有效的药物。
Objective: To study the preventive effect of drug treatment on the adverse pregnancy outcome of bacterial vaginosis (BV) in late pregnancy. Methods: The BV screening of pregnant women of 28 ~ 36 weeks of gestation from January 2006 to January 2007 in our hospital was conducted. The 150 BV patients screened to meet the standard of care were randomly divided into Fu Jie lotion group, Metronidazole suppository group, Lactobacillus live capsule group, each group of 50 cases. Metronidazole suppository group, Lactobacillus live bacteria capsule group were a drug vaginal administration, Fu Jie lotion group Fu Jie washing lotion 20ml water wash the vulva, were discontinued for 10 days, 1 week after the review. Results: There were 50 cases in Fujie lotion group and no one case was cured. 50 cases in metronidazole suppository group and 45 cases were cured, 2 cases were markedly effective, the total effective rate was 94%. Lactobacillus live capsule group was 50 cases and 40 cases were cured , 4 cases markedly effective, the total efficiency of 88%. Metronidazole suppository group and Lactobacillus live bacteria capsule group in the treatment effect, no significant difference between the two groups (P> 0.05). Preterm birth, premature rupture of membranes, premature rupture of membranes, low birth weight infants, neonatal jaundice, metronidazole suppository group and Lactobacillus live capsule group were significantly lower than the cleansing lotion group, the difference was significant (P <0.05). Conclusion: The metronidazole suppository and Lactobacillus live capsule have a significant preventive effect on the adverse pregnancy outcome in the third trimester of pregnancy. It is a safe and effective drug for the treatment of BV in the third trimester of pregnancy.
