正鑫一消贴+疼痛定向透疗仪联合三阶梯方案缓解癌性疼痛随机平行对照研究

[目的]观察正鑫一消贴+疼痛定向透疗仪联合三阶梯方案缓解癌性疼痛疗效。[方法]使用随机平行对照方法,将120例住院患者按随机数字表法随机分为两组。对照组60例三阶梯治疗:轻度疼痛给与布洛芬胶囊,0.3g/次,1次/6h,口服。中度疼痛给与氨酚曲马多片,37.5mg/次,3次/d,口服。重度疼痛给与盐酸吗啡缓释片,30mg/次,2次/d,口服。治疗组60例在病灶区、穴位及疼痛点贴敷正鑫一消贴,2贴/d,1贴/12h,贴敷后针对该穴(区)直接予疼痛定向透疗仪照射,20min/次。每4d用数字分级法自测疼痛强度及药物副作用。两组均连续治疗7d为1疗程。观测临床症状、疼痛程度分级、疼痛缓解度、生活质量评分、不良反应。连续治疗2疗程,判定疗效。结果:镇痛效果治疗组Ⅰ度3例,Ⅱ度5例,Ⅲ度20例,Ⅳ度20例,Ⅴ度12例,缓解率86.60%。对照组Ⅰ度5例,Ⅱ度20例,Ⅲ度15例,Ⅳ度15例,Ⅴ度5例,缓解率58.30%。治疗组疗效优于对照组(P<0.01)。生活质量评分治疗组优于对照组(P<0.05)。结论:正鑫一消贴+疼痛定向透疗仪联合三阶梯方案缓解癌性疼痛效果显著,值得推广。 [Objective] To observe the curative effect of Zhengxin-Hypothesis and pain-directed radiotherapy apparatus combined with three-step ladder to relieve the cancer pain. [Methods] Using randomized parallel method, 120 inpatients were randomly divided into two groups according to random number table method. The control group of 60 cases of three-step treatment: mild pain given ibuprofen capsules, 0.3g / time, 1 / 6h, orally. Moderate pain given trastuzumab tablets, 37.5mg / time, 3 times / d, orally. Severe pain given morphine hydrochloride sustained-release tablets, 30mg / times, 2 times / d, orally. The treatment group, 60 cases in the lesion area, acupoints and pain points affixed Xin a paste, 2 paste / d, 1 paste / 12h, after the application for the hole (area) directly to the pain directed radiotherapy instrument, 20min / Times. Self-rated pain intensity and drug side effects with digital grading every 4 days. Both groups were treated for 7 days for a course of treatment. Clinical symptoms, grading of pain, pain relief, quality of life score, and adverse reactions were observed. Continuous treatment of 2 courses to determine the efficacy. Results: The analgesic effect in treatment group Ⅰ 3 cases, Ⅱ 5 cases, Ⅲ 20 cases, Ⅳ 20 cases, Ⅴ degree in 12 cases, the response rate was 86.60%. In control group, there were 5 cases of grade Ⅰ, 20 cases of degree Ⅱ, 15 cases of degree Ⅲ, 15 cases of degree Ⅳ and 5 cases of degree Ⅴ. The remission rate was 58.30%. The treatment group than the control group (P <0.01). Quality of life scores in the treatment group were better than those in the control group (P <0.05). Conclusion: Zhengxin a paste + pain-directed radiotherapy apparatus combined with three-step solution to relieve the painful effect of cancer is worthy of promotion.
