目的:了解北京城区某单位儿童群体的健康状况。方法:2019年10—11月对北京城区某单位职工的558例0~13岁子女进行健康体检,项目包括身高、体重、耳鼻咽喉科、心肺等。对体格状况和检出率较高的项目进行统计分析,将体质指数(BMI)≥Pn 85(P为与标准化生长曲线相比的百分位数)定为超重、≥Pn 95定为肥胖。用率和构成比来描述儿童体检各项目相关数据,用χ2检验比较不同年龄组、性别组间各指标的差异。n 结果:检出率较高的项目主要有:超重、肥胖、鼻炎、龋齿、包茎等。儿童体重、身高≤Pn 10的比率分别是3.4%和2.7%,≥Pn 97的比率分别是12.0%和7.0%,儿童超重和肥胖率分别是13.8%和13.3%;儿童总体和男童中体重≥Pn 97、身高≥Pn 97、BMI≥Pn 95比率均随年龄增长而增加(均n P<0.05),男童中BMI≤Pn 5比率亦随年龄增长而增加(n P<0.05),但体重≥Pn 75比率却随年龄增长而减少(n P<0.05);男童中体重≥Pn 75比率显著多于女童(n P<0.05)。儿童中鼻炎患病率为39.8%,其中过敏性鼻炎患病率为14.9%;儿童总体3.0~5.9岁组过敏性鼻炎患病率显著高于0.0~0.9岁、6.0~9.9岁组(均n P<0.05),女童3.0~5.9岁组总鼻炎患病率均显著高于0.0~0.9岁、1.0~2.9岁、10.0~13.9岁组(均n P<0.05)。儿童龋齿患病率(剔除0.0~0.9岁组)为26.0%,儿童总体3.0~5.9岁组龋齿患病率显著高于1.0~2.9岁、10.0~13.9岁组(均n P<0.05),男童、女童的3.0~5.9岁和6.0~9.9岁组龋齿患病率均显著高于1.0~2.9岁组(均n P<0.05)。男童包茎率为31.5%,1.0~2.9岁、3.0~5.9岁组比率均显著高于6.0~9.9岁、10.0~13.9岁组(均n P<0.05)。未发现肥胖与非肥胖儿童的过敏性鼻炎患病率、龋齿患病率差异有统计学意义。n 结论:儿童超重肥胖严重程度超出预期,过敏性问题日益凸显,龋齿防治亟待改进,男童包茎问题有待关注。“,”Objective:To understand the health status of children in an organization in the urban area of Beijing.Methods:From October to November 2019, 558 children aged from 0 to 13 of employees in an organization in Beijing received physical examination, including height, weight, ophthalmology and otorhinolaryngology, heart and lung examination, etc.Statistical analysis was conducted on the items related to physical status and with higher detection rate. Body mass index (BMI)≥Pn 85(P is the percentile compared to the standardized growth curve) was defined as overweight and BMI≥Pn 95 as obese. The rate and composition ratio were used to describe the relevant data of each item in children′s physical examination. The differences between different age groups and gender groups were compared with the chi-square test.n Results:The items with high detection rate were overweight, obesity, rhinitis, caries, phimosis, etc. The ratio of children′s weight and height ≤Pn 10 were 3.4% and 2.7%, ≥Pn 97 were 12.0% and 7.0%respectively; the rates of overweight (BMI≥Pn 85) and obesity (BMI≥Pn 95) in children were 13.8% and 13.3% respectively, the ratio of weight ≥Pn 97, height ≥Pn 97 and BMI≥Pn 95all increased with age in all children and in boys (all n P<0.05), the ratio of BMI≤Pn 5 increased with age (n P<0.05), and the ratio of body weight ≥Pn 75 decreased with age in boys (n P<0.05); the ratio of weight ≥Pn 75 in boys was significantly higher than that in girls (n P<0.05). The prevalence of rhinitis in children was 39.8%, among which the prevalence of allergic rhinitis was 14.9%.The prevalence of allergic rhinitis in the 3.0-5.9 age group of all children was significantly higher than that in 0.0-0.9 age group and 6.0-9.9 age group (bothn P<0.05), the prevalence of total rhinitis in the 3.0-5.9 age group of girls was significantly higher than that in 0.0-0.9 age group, 1.0-2.9 age group and 10.0-13.9 age group (alln P<0.05). The prevalence of dental caries in children (excluding the group of 0.0-0.9 years old) was 26.0%, the prevalence of dental caries in the 3.0-5.9 age group of all children was significantly higher than that in 1.0-2.9 age group and 10.0-13.9 age group (bothn P<0.05), the prevalence of dental caries in 3.0-5.9 age group and 6.0-9.9 age group of boys and girls were both significantly higher than that in 1.0-2.9 age group (alln P<0.05). The prevalence of phimosis in boyswas 31.5%, the phimosis rates were significantly higher in the 1.0-2.9 age group and 3.0-5.9 age group than that in 6.0-9.9 age group and 10.0-13.9 age group (alln P0.05).n Conclusion:The severity of overweight and obesity in children is beyond our expectation, the allergenicity problem is becoming more and more prominent, the prevention and treatment of dental caries needs to be improved urgently, and the phimosis problem in boys needs to be paid attention to.
