目的建立一种创伤失血性休克并全身炎症反应综合征(SIRS)犬模型,满足多器官功能障碍综合征(MODS)实验研究的需要。方法选择15只杂种犬,用重量为1.5kg的铁块从18cm高处自由落下分别锤击每只犬的两侧小腿造成双侧胫腓骨粉碎性骨折,再用同法在每侧小腿骨折段局部增加锤击9次,并从股动脉导管放血至平均动脉血压(MAP)40~55mmHg维持1h;检测休克后1h犬的主要生理指标有两项或以上符合动物SIRS标准者作为SIRS动物模型;模型犬复苏至休克后24h,统计休克后24h实验犬的MODS发生率。结果平均每只犬的失血量为血液总量的38.9%,MAP较基础值下降了63%;15只犬休克后1h均达到动物SIRS标准,其中符合SIRS指标4项的1只3、项的5只、2项的9只;休克后24hMODS发生率为33.3%的。结论本法制作的创伤失血性休克模型能达到稳定的SIRS动物标准,可为开展MODS的实验研究应用。
Objective To establish a canine model of traumatic hemorrhagic shock and systemic inflammatory response syndrome (SIRS) to meet the needs of MODS experiments. Methods Fifteen mongrel dogs were randomly selected and dropped freely from a height of 18cm with a weight of 1.5kg of iron. The two sides of each dog were shattered to cause bilateral tibiofibular comminuted fractures respectively. Local increased hammering 9 times, and from the femoral artery to the arterial blood pressure (MAP) 40 ~ 55mmHg maintain 1h; 1h after the test dog major physiological indicators of two or more animals in line with SIRS standards as SIRS animal model; The model dogs were resuscitated 24h after shock, and the incidence of MODS in experimental dogs 24 h after shock was calculated. Results On average, the blood loss per dog was 38.9% of the total blood and the MAP was 63% lower than the baseline values. The animal SIRS standards were achieved at 1 hour after shock in 15 dogs, of which 1 in 3 of 4 items of SIRS index 5, 2 of 9; 24hMODS after shock was 33.3%. Conclusion The traumatic hemorrhagic shock model produced by this method can reach the stable SIRS animal standard and can be used for the experimental research on MODS.
