目的探讨腹腔镜技术在飞行人员中的临床应用。方法采用德国Storz公司产的电视腹腔镜及其配套设备,0°或30°镜。对30例飞行人员(歼-7飞行员3例,歼-6飞行员1例,歼教机飞行员5例,直升机飞行员2例,民航驾驶员10例,空中乘务员7例,空中机械师1例,领航员1例)患者实施胆囊切除术24例,胆囊与阑尾联合切除3例,胆囊切除加肝囊肿开窗引流2例,胆囊切除加左侧精索静脉曲张高位结扎1例。结果30例手术均获成功,无中转开腹及并发症发生。平均手术时间26min,术后4~10h下地活动,12~24h进流食。创口疼痛较轻,术后平均住院6.5天。地面观察及疗养三个月,3例因其他原因停飞,其余27例均飞行合格。随访8~28个月,健康状况良好,各课目飞行均未见特殊不良反应。结论腹腔镜技术可应用于飞行人员,且能缩短术后恢复的时限。但应选择相应的适应证及手术时机,注意围手术期处理及组织技术准备。
Objective To explore the clinical application of laparoscopy in pilots. Methods TV laparoscope and its related equipment produced by Storz Company of Germany, 0 ° or 30 ° mirror were used. A total of 30 pilots (3 F-7 pilots, 1 F-6 pilot, 5 F-pilots, 2 helicopter pilots, 10 pilots, 7 flight attendants, 1 air mechanic pilot, pilot pilot 1 patient) underwent cholecystectomy in 24 patients, gallbladder and appendix combined resection in 3 cases, cholecystectomy plus hepatic cyst fenestration in 2 cases, cholecystectomy plus left varicocele in 1 case. Results All the 30 surgeries were successful without conversion to laparotomy and complications. The average operation time 26min, 4 ~ 10h after surgery, 12 ~ 24h into the flow of food. Wound pain less, average postoperative hospital stay of 6.5 days. Three months of ground observation and recuperation, three stopped for other reasons, and the remaining 27 were eligible for flight. All cases were followed up for 8 ~ 28 months and were in good health. No special adverse reactions were observed in all the subjects. Conclusion Laparoscopic technique can be applied to pilots and can shorten the time limit for postoperative recovery. But should choose the appropriate indications and timing of surgery, pay attention to perioperative management and organizational technical preparation.
