目的:在新疆阿合奇县医院开展彩超检测脐血流,探讨彩色多普勒脐血流分析仪在高危妊娠中监护胎儿宫内安危的临床价值。方法:检测2014年5月至2015年5月新疆阿合奇县人民医院住院,妊娠30~42+5周孕妇的胎儿脐动脉血流频谱,测定S/D比值(收缩期峰值与舒张期低值之比)。分析异常组(S/D≥3)25例及正常组40例胎儿的妊娠结局。结果:异常组中重度子痫前期,脐带绕颈,胎儿生长受限(F G R)及胎儿窘迫的发生率均显著高于正常组。结论:高危妊娠胎儿由于胎盘和脐带的因素导致脐带血流缓慢,供血供氧较正常胎儿差,应用彩超测量脐血流可表现为S/D异常,S/D异常与胎儿窘迫呈正相关。目前全国各地县级以上医院已将孕晚期彩超检测脐血流作为常规检查项目之一。我们阿合奇也应该充分利用先进的仪器和设备,逐步开展新技术新项目,缩短与发达地区的差距,更好地为广大农牧民服务。“,”Objective In Xinjiang Akeqi County People’s Hospital, we carry out color Doppler flow imaging diagnosis instrument (CDFI) examinations on umbilical artery blood flow so that we can evaluate the significance of Doppler umbilical artery systolic / diastolic (S / D) ratio examination on monitoring the fetal status in the uterus during the high risk pregnancy.Methods We have carried out color Doppler umbilical artery S / D ratio examinations on the pregnant women with 30 ~ 42 + 5 weeks of pregnancy in our hospital from May, 2014 to May, 2015. Do analysis on the birth results of 25 cases in the abnormal group (S / D≥3) and 40 cases in the normal group.Results The rates of moderate and severe preeclampsia, umbilical cord entanglement around fetal neck, fetal growth restriction (FGR) and fetal hypoxia in the abnormal group are all notably higher than the normal group.Conclusion The umbilical artery blood flow of the fetus in high risk pregnancy can be really slow because of placenta and umbilical cord. These fetuses’ supplies of blood and oxygen can be worse than the normal ones. The color Doppler umbilical artery S / D ratio results can be abnormal, which has positive correlation with the fetal hypoxia. Nowadays the color Doppler umbilical artery S/ D ratio examination during the last trimester of pregnancy has been one of the routine checks in the hospitals above the county level in China. Akeqi County People’s Hospital also should make full use of our advanced instrumentation to gradually carry out the new examinations of the new technologies to narrow the gap between Akeqi and developed areas and provide the local herdsmen with better medical care.
