目的探讨体外循环过程中的各种因素对血清地高辛浓度的影响。方法将可能影响血清地高辛浓度的各种因素进行多因素的逐步回归分析,其结果与临床综合考虑,从而探讨影响地高辛浓度的诸因素。结果术前血清地高辛浓度与血清钾、镁离子浓度、服药剂量及肌酐清除率有关;体外循环结束时的血清地高辛浓度与术中的尿量与超滤量、停跳液及预充液量、术前地高辛的用药剂量有关;术后4小时的地高辛血清浓度与术后尿量有关。结论地高辛的血清浓度在体外循环不同时段的变化与不同因素相关。
Objective To investigate the effects of various factors during cardiopulmonary bypass on serum digoxin concentration. Methods Various factors that may affect serum digoxin concentration were analyzed by stepwise multiple regression analysis. The results were integrated with clinical considerations to explore the factors that affect the concentration of digoxin. Results Preoperative serum digoxin concentration and serum potassium and magnesium ion concentrations, medication dose and creatinine clearance rate; end of the cycle of serum digoxin concentration and intraoperative urine output and ultrafiltration volume, stopping fluid and pre Fluid volume, preoperative digoxin dose related; after 4 hours of digoxin serum concentration and postoperative urine volume related. Conclusion The changes of digoxin serum concentration in different periods of cardiopulmonary bypass are related to different factors.
