目的探讨基质金属蛋白酶-2(MMP-2)及其组织抑制剂(TIMP-2)在人肝癌组织中的表达和生物学特性。方法收集手术切除、术前未行其他治疗且临床病理资料完整的54例肝细胞癌(HCC)患者的石蜡组织标本作研究对象,5例因肝外伤切除的正常肝脏组织作为对照组。对石蜡标本进行连续切片,应用免疫组织化学SP法分别检测HCC组织及正常肝组织中MMP-2、TIMP-2的表达情况。结果MMP-2、TIMP-2的阳性表达定位于癌细胞的细胞浆中,呈棕黄色细颗粒状。HCC组织中MMP-2的阳性表达率为75.9%,TIMP-2的阳性表达率为38.9%,MMP-2的阳性表达率高于TIMP-2(P<0.05)。MMP-2表达与病理分期、病理分级和有无门静脉癌栓有关;TIMP-2表达与病理分期、病理分级有关,与有无门静脉癌栓无关。在包膜完整、包膜突破及无包膜组,HCC组织MMP-2的阳性表达率略呈递降,但差异无统计学意义(P>0.05);而TIMP-2的表达率呈递降趋势,且差异有统计学意义(P<0.05)。正常组织中MMP-2和TIMP-2未见阳性表达。结论MMP-2与TIMP-2的表达状态对判断HCC的侵袭转移有重要价值。
Objective To investigate the expression and biological characteristics of matrix metalloproteinase-2 (MMP-2) and tissue inhibitor of metalloproteinase-2 (TIMP-2) in human hepatocellular carcinoma. Methods Paraffin-embedded specimens from 54 patients with hepatocellular carcinoma (HCC) who had undergone surgical resection and no other treatment before surgery were enrolled. Five normal liver tissues resected from liver injury were selected as the control group. The paraffin specimens were serially sectioned, and the expression of MMP-2 and TIMP-2 in HCC tissues and normal liver tissues were detected by immunohistochemical SP method. Results The positive expression of MMP-2 and TIMP-2 localized in the cytoplasm of cancer cells and showed the shape of brownish yellow granules. The positive expression rate of MMP-2 in HCC tissues was 75.9%, the positive expression rate of TIMP-2 was 38.9%, and the positive expression rate of MMP-2 was higher than that in TIMP-2 (P <0.05). The expression of MMP-2 was related to the pathological stage, pathological grade and portal vein tumor thrombus. The expression of TIMP-2 was related to the pathological stage and histological grade, but not to the presence or absence of portal vein tumor thrombus. The positive expression rate of MMP-2 in HCC tissue decreased slightly, but the difference was not statistically significant (P> 0.05). The expression rate of TIMP-2 showed a decreasing trend, And the difference was statistically significant (P <0.05). Normal tissues showed no positive expression of MMP-2 and TIMP-2. Conclusions The expressions of MMP-2 and TIMP-2 are of great value in judging the invasion and metastasis of HCC.
