目的:为了在前庭蜗神经瘤(VN)手术中提高面神经和前庭蜗神经功能保留率。方法:在手术显微镜下观察了52例前庭蜗神经瘤病人的桥脑小脑角(CPA)解剖结构及VN与周围神经、血管的解剖关系。结果:在VN包膜外两层蛛网膜间粘连最少,容易解剖分离,有利于术中保留面、前庭蜗神经功能。结论:在手术显微镜下能更清晰地辨清VN周围的解剖关系,有助于减少面、前庭蜗神经的损伤。
OBJECTIVE: To improve the functional retention of the facial nerve and vestibular nociception in the treatment of vestibular nocturnal neuroma (VN). Methods: The anatomic structures of the pontine cerebellopontine angle (CPA) of 52 cases of vestibular meningioma were observed under an operative microscope. The anatomic relationships between VN and peripheral nerves and blood vessels were observed. Results: There was the least adhesions between the two layers of arachnoid outside the capsule of VN, which was easy to dissect and separate. It is helpful for preserving the functions of facial and vestibular cochlear nerves. Conclusion: The anatomical relationship around the VN can be distinguished more clearly under the operating microscope, which helps to reduce the damage of the facial and vestibular cochlear nerves.
