目的:利用3.0T动态对比增强磁共振(dynamic contrast-enhanced magnetic resonance,DCE-MR),探讨直肠癌患者容量转移常数(volume transfer constant,Ktrans)、速率常数(rate constant,Kep)以及细胞外血管外间隙容积比(extra-vascular extra-cellular space fractional volume,Ve)与肿瘤组织分化程度的关系。材料与方法:根据术后病理,56例直肠癌患者分为高、中、低分化组,利用单因素方差分析比较各组Ktrans、Kep、Ve值。结果:高、中、低分化组的Ktrans值分别为0.154±0.009/min、0.210±0.027/min、0.365±0.021/min;Kep值分别为0.469±0.008/min、0.528±0.080/min、0.637±0.006/min;Ve值分别为0.739±0.006、0.841±0.070、0.923±0.105,高、中、低分化组间Ktrans、Kep、Ve差异均有统计学意义(P<0.01)。结论:DCE-MR定量参数Ktrans、Kep、Ve值可提供肿瘤微循环信息,并且能够无创性预测直肠癌的恶性程度。
Objective: To investigate the relationship between volume transfer constant (Ktrans), rate constant (Kep) and extracellular blood vessels in patients with rectal cancer using 3.0T dynamic contrast-enhanced magnetic resonance (DCE-MR) Relationship between extra-cellular extracellular space fractional volume (Ve) and tumor differentiation. Materials and Methods: According to the postoperative pathology, 56 patients with rectal cancer were divided into high, medium and poorly differentiated groups. Ktrans, Kep and Ve values of each group were compared by one-way analysis of variance. Results: The Ktrans values of high, moderate and poorly differentiated groups were 0.154 ± 0.009 / min, 0.210 ± 0.027 / min and 0.365 ± 0.021 / min, respectively. Kep values were 0.469 ± 0.008 / min, 0.528 ± 0.080 / min and 0.637 ± 0.006 / min and Ve values were 0.739 ± 0.006, 0.841 ± 0.070 and 0.923 ± 0.105, respectively. There were significant differences in Ktrans, Kep and Ve between high, medium and low differentiation groups (P <0.01). CONCLUSION: The Ktrans, Kep, Ve values of DCE-MR quantitative parameters can provide tumor microcirculation information and can predict the malignancy of rectal cancer noninvasively.
