目的：比较内固定和人工全髋关节置换术治疗老年股骨颈囊内骨折的效果差异，以探讨老年移位型股骨颈囊内骨折的治疗方法选择的依据。方法自2000年1月～2012年1月，95例老年移位型股骨颈囊内骨折的患者中47例行内固定术，48例采用人工全髋关节置换术。髋关节功能评价根据Harris评分法进行评分。结果本组95例患者随访13～45个月，平均（22．2±3．4）个月。内固定组髋关节功能Harris评分为（80．2±3．4），全髋置换组为（96．2±2．6），内固定组髋关节功能Harris评分低于全髋置换组，两组比较差异有统计学意义（P＜0．05）。结论人工全髋关节置换术治疗老年移位型股骨颈囊内骨折优于内固定术。“,”Objective To compare the curative ef ect of intracapsular fractures of the femoral neck in elderly patients treated with internal fixation or total hip replacement.Methods From Jan.2000 to Jan.2012,Ninety five old patients with displaced intracapsular fractures of the femoral neck were treated with dif erent methods,and 47 cases were underwent internal fixation;48 cases underwent total hip replacement.The hip function assessment was scored according to Harris methods,whereas the synthetic evaluation included surgery time,intraoperative blood loss, blood transfusion,mean hospital stay,out of bed time,weight bearing time,total medical cost,reoperation,and complication and so on.Results Al patients were fol owed up for a mean of (22.2 ±3.4)months (ranging from 13 to 45 months).The Harris score was (80.2 ±3.4)in internal fixation group,and (96.2 ±2.6)the total hip replacement group.The Harris score was lower in internal fixation group ,compared with the total hip replacement group(P<0.05).Conclusion The treatment efect of total hip replacement on displaced intracapsular fractures of the femoral neck in elderly patients is bet er than that of internal fixation.
