过敏性紫癜是一种免疫复合物性血管炎,临床上以皮肤紫癜伴关节、胃肠道和肾脏的表现为特征,与各种感染因素有一定的关系。据了解,过去文献中仅报导过1例成人病人,其发病可能与乙型肝炎病毒(HBV)感染有关。本文作者报告2例儿童,其紫癜等损害发生在HBV感染的过程中。例1,男,8岁。因持续性蛋白尿及血尿而住院。3个月前曾因下肢、臀部紫癜,伴膝、腕、肘关节肿胀、压痛而在其他医院住院。近日有腹绞痛、血便,尿检查有明显的蛋白尿和血尿。实验室检查发现丙氨酸转移酶(ALT)水平升高,HBsAg阳性。
