目的探讨CD19+B淋巴细胞、CD56+CD16+NK细胞和补体C3、C4水平在壹期煤工尘肺(CWP)中的变化及意义。方法采用流式细胞术检测56例壹期CWP患者(CWP组)、45例井下健康接尘矿工(接尘对照组)及48例井上健康人(正常对照组)外周血中CD19+B淋巴细胞、CD56+CD16+NK细胞表达水平;免疫比浊法检测血清补体C3、C4水平。结果与正常对照组和接尘对照组比较,CWP组CD19+升高,CD56+CD16+下降,差异有统计学意义(P<0.05或P<0.01);C3与C4水平无明显改变(P>0.05)。接尘对照组CD56+CD16+和C4水平均高于正常对照组,差异有统计学意义(P<0.05);CD19+和C3水平虽也有所升高,但差异无统计学意义(P>0.05)。CWP组C3与C4呈正相关(r=0.610,P<0.01),其他各指标间无相关性。结论壹期CWP患者存在B淋巴细胞介导的体液免疫上调,NK细胞免疫功能低下。
Objective To investigate the changes and significance of CD19 + B lymphocytes, CD56 + CD16 + NK cells and complement C3 and C4 in coal worker’s pneumoconiosis (CWP). Methods Flow cytometry was used to detect CD19 + B lymphocytes in peripheral blood of 56 patients with CWP (CWP group), 45 healthy miners (dust control group) and 48 healthy people (normal control group) , CD56 + CD16 + NK cells were detected by enzyme-linked immunosorbent assay (ELISA). Serum levels of complement C3 and C4 were detected by immunonephelometry. Results The levels of CD19 + and CD56 + CD16 + in CWP group were significantly lower than those in normal control group and control group (P <0.05 or P <0.01). There were no significant changes in C3 and C4 levels (P> 0.05) . The levels of CD56 + CD16 + and C4 in the dust-exposed control group were significantly higher than those in the normal control group (P <0.05). The levels of CD19 + and C3 were also increased, but the differences were not statistically significant (P> 0.05). There was a positive correlation between C3 and C4 in CWP group (r = 0.610, P <0.01). There was no correlation between other indexes. Conclusions There is an upregulation of B lymphocyte-mediated humoral immunity in one-stage CWP patients and a low immune function of NK cells.
