本研究旨在探讨特发性血小板减少性紫癜(ITP)中血小板特异性糖蛋白抗体和HLA抗体的表达。选择45例ITP患者,运用ELISA方法检测患者的血浆和血小板洗脱物血小板特异性糖蛋白抗体和HLA抗体。使用HPA分型试剂盒检测7个抗原系统HPA-1、2、3、4、5、6、15。结果表明,有45例患者检测到血清或者血小板洗脱物中存在血小板特异性糖蛋白抗体,其中以抗GPⅡb/Ⅲa/HIa和抗GpⅠb/Ⅸ最为常见;有11例患者同时表达HLA抗体和血小板特异性糖蛋白抗体。对病例37和40进行家系调查,发现这些病例所产生的血小板特异性糖蛋白抗体和HLA抗体与父母血小板抗原及HLA抗原不相合密切相关。结论:应用ELISA检测ITP病例的血浆和血小板洗脱物,并与病人的临床表现相结合,对提高ITP的诊断具有重要价值。
This study aimed to investigate the expression of platelet-specific glycoprotein and HLA antibodies in idiopathic thrombocytopenic purpura (ITP). 45 patients with ITP were selected and the plasma and platelet-specific platelet-specific glycoprotein and HLA antibodies were detected by ELISA. Seven antigen systems, HPA-1, 2, 3, 4, 5, 6, 15 were tested using the HPA typing kit. The results showed that there were 45 cases of platelet-specific glycoprotein antibodies detected in serum or platelet eluate, of which GPⅡb / Ⅲa / HIa and anti-GpⅠb / Ⅸ were the most common, and 11 cases expressed both HLA antibodies and platelets Specific glycoprotein antibody. A pedigree study of cases 37 and 40 found that the platelet-specific glycoprotein and HLA antibodies produced in these cases were closely related to the mismatch between parental platelet antigens and HLA antigens. CONCLUSIONS: The use of ELISA for the detection of plasma and platelet eluates in patients with ITP, combined with the clinical presentation of the patient, is of great value in the diagnosis of ITP.
