目的探讨过敏性紫癜患儿经抗过敏治疗后早期外周血T淋巴细胞上PD-1分子的表达变化及其与体液免疫IgA的关系。方法应用荧光抗体染色技术经流式细胞仪动态分别检测20例健康对照组和40例过敏性紫癜患儿急性期和经抗过敏治疗后早期外周血T淋巴表面的CD4+和CD8+T细胞表面的PD-1分子的表达水平,同时应用免疫比浊法查血清IgA水平,探讨其表达与血清IgA水平之间的关系。结果过敏性紫癜患儿治疗前急性期和与健康对照组相比,外周血T淋巴表面的CD4+T细胞组PD-1在外周血淋巴细胞阳性表达率明显升高,差异有统计学意义(P<0.01);外周血T淋巴表面的CD8+T细胞组PD-1在外周血淋巴细胞阳性表达率升高,差异有统计学意义(P<0.05);同时,血清IgA水平升高,差异有统计学意义(P<0.05);治疗后与健康对照组相比,差异无统计学意义(P>0.05)。结论有效的抗过敏治疗可使过敏性紫癜患儿急性期外周血清IgA水平逐渐降低,T淋巴细胞表面PD-1表达下降,推测IgA水平与T淋巴细胞表面表面PD-1表达水平变化与患儿免疫调节机制紊乱有关。
Objective To investigate the expression of PD-1 on peripheral blood T lymphocytes in allergic purpura patients after anti-allergic treatment and its relationship with IgA in humoral immunity. Methods Fluorescent antibody staining was used to detect the expression of CD4 + and CD8 + T cells on T lymphocytes in peripheral blood of 20 healthy controls and 40 cases of Henoch-Schonlein purpura in acute and anti-allergic patients respectively by flow cytometry. PD-1 molecule expression level, while the application of immune turbidimetry serum IgA levels to explore the relationship between its expression and serum IgA levels. Results The positive expression rate of PD-1 in peripheral blood lymphocytes of CD4 + T cells in peripheral blood T lymphocytes was significantly higher in acute pre-treatment children and children with Henoch-Schonlein purpura than in healthy controls P <0.01). The positive rate of PD-1 in peripheral blood mononuclear cells of peripheral blood T lymphocytes increased significantly in peripheral blood lymphocytes (P <0.05), meanwhile, serum IgA level increased (P <0.05). Compared with healthy control group, there was no significant difference after treatment (P> 0.05). Conclusions Effective anti-allergy treatment can reduce the levels of IgA in peripheral blood of children with HSP, and decrease the expression of PD-1 on T lymphocytes. It is speculated that the level of IgA and the expression of PD-1 on the surface of T lymphocytes are similar to those in children Immune regulation mechanism disorder related.
