目的:研究抗核小体抗体(AnuA)在系统性红斑狼疮疾病(SLE)治疗前后的关系。方法:选取2012年1月到2012年12月,经上一级医院确诊为SLE的患者作为研究对象,用欧蒙的抗核抗体谱印迹法条带检测患者治疗前后的AunA、抗Sm抗体,同时用欧蒙的间接免疫荧光法检测SLE患者的抗ds-DNA抗体。依据治疗前后各个指标的变化,筛选出与系统性红斑狼疮疾病活动相关性较强指标。结果:SLE患者中的AnuA的阳性率为89%,而其他抗ds-DNA抗体以及血清Sm抗体的阳性率分别为66%,63%,同时,AnuA、抗ds-DNA抗体、抗Sm抗体在SLE组患者敏感性和特异性分别为70.4%、49.8%、43.1%和90%、86%、88%。因此AnuA在SLE患者中具有较高的阳性率、敏感性以及特异性,对于诊断SLE有着重要的意义。治疗后抗ds-DNA抗体以及血清Sm抗体显示阳性患者的数量虽然于治疗前有所下降,但是差异无统计学意义(P>0.05),但是AunA显示阳性患者的数量相比于治疗前显著下降(P<0.05)。结论:抗核小体抗体(AnuA)与系统性红斑狼疮疾病具有一定的相关性,可以作为诊断和治疗SLE患者的重要指标,有实际的临床意义。
Objective: To study the relationship between anti-nucleosome antibody (AnuA) before and after treatment of systemic lupus erythematosus (SLE). Methods: From January 2012 to December 2012, the patients diagnosed as SLE in the upper level hospitals were selected as the research objects. AunA and anti-Sm antibodies before and after treatment were detected by the banding of the anti-nuclear antibody McAb of Aumon, At the same time, the anti-ds-DNA antibody of patients with SLE was detected by indirect immunofluorescence assay. Based on the changes of various indexes before and after treatment, we screened out the strong correlation with the disease activity of systemic lupus erythematosus. Results: The positive rate of AnuA in SLE patients was 89%, while the positive rates of other anti-dsDNA antibodies and serum Sm antibodies were 66% and 63% respectively. Meanwhile, AnuA, anti-dsDNA and anti-Sm antibodies The sensitivity and specificity of SLE patients were 70.4%, 49.8%, 43.1% and 90%, 86% and 88% respectively. Therefore, AnuA in SLE patients with high positive rate, sensitivity and specificity, for the diagnosis of SLE is of great significance. After treatment, the number of positive anti-dsDNA antibody and serum Sm antibody showed no significant difference (P> 0.05), but the number of positive patients with AunA showed a significant decrease compared with that before treatment (P <0.05). Conclusion: Anti-nucleosome antibody (AnuA) has some correlation with systemic lupus erythematosus (SLE) and may be used as an important index in the diagnosis and treatment of SLE patients. It has practical clinical significance.
