目的:探讨健脾益肺方治疗老年稳定期慢性阻塞性肺疾病(COPD)的疗效及对患者免疫功能的影响.方法:选取90例COPD稳定期者为研究对象,采用随机数字表法分为对照组和治疗组,各45例.两组均予常规西医对症治疗,治疗组再予健脾益肺方治疗.观察不同干预后两组患者肺功能、生存质量、免疫功能、远期疗效的变化.结果:两组治疗后FEV1、FEV1/FVC均高于同组治疗前(P＜0.05),而生存质量评分低于同组治疗前(P＜0.05);治疗组治疗后FEV1、FEV1/FVC均高于同期对照组(P＜0.01),而生存质量评分低于同期对照组(P＜0.01).治疗后治疗组CD4+、CD4+/CD8+水平均较同组治疗前升高,而CD8+水平则下降,组内差异均有统计学意义(P＜0.05);治疗后对照组CD4+水平高于同组治疗前(P＜0.05),而CD8+、CD4+/CD8+治疗前后组间差异无统计学意义(P＞0.05);治疗后治疗组CD4+、CD4+/CD8+水平高于同期对照组(P＜0.01),而CD8+水平低于同期对照组(P＜0.01).两组患者治疗观察过程中急性发作次数、急诊次数均低于对照组(P＜0.01).对照组总有效率73.3％,治疗组91.1％,组间差异有统计学意义(P＜0.05).结论:健脾益肺方治疗老年稳定期慢性阻塞性肺疾病疗效确切,可有效改善患者肺功能及生存质量,远期疗效好,可能与提高患者免疫功能相关.“,”Objective:To investigate the effect of Jianpi Yifei prescription on chronic obstructive pulmonary disease (COPD) and its effect on immune function.Methods:The 90 patients with stable COPD were randomly divided into control and treatment groups,45 cases in each.The two groups were given conventional western medicine symptomatic treatment;the treatment group was treated with Jianpi Yifei Prescription.The changes of lung function,quality of life,immune function and long-term curative effect were observed in the two groups after intervention.Results:The FEV1 and FEV1/FVC increased in the two groups after treatment (P＜0.05),while the quality of life scores decreased after treatment (P＜0.05);the treatment group showed higher FEV1 and FEV1/FVC than the control group after treatment (P＜0.01),while lower life quality score than the control group (P＜0.01).The level of CD4+ and CD4+/CD8+ increased and the level of CD8+ decreased in treatment group after treatment (P＜0.01).The level of CD4+ increased in control group after treatment (P＜0.05),while the changes of CD8+ and CD4+/CD8+ were not statistically significant (P＜0.05);The treatment group showed higher level of CD4+ and CD4+/CD8+ than the control group (P＜0.01),while lower level of CD8+ than the control group (P＜0.01).The treatment group showed lower acute exacerbation of the number of times,the number of emergency treatment,medical costs than the control group (P＜0.01).The total effective rate was 73.3％ in the control group and 91.1％ in treatment group;the difference between the two groups is statistically significant (P＜0.05).Conclusion:Jianpi Yifei Prescription is effective in the treatment of elderly patients with stable COPD,which can effectively improve the lung function and quality of life.
