目的:检测microRNA-16(miR-16)在多发性骨髓瘤(MM)患者骨髓单个核细胞(BMMNC)中表达水平,分析miR-16表达水平与Durie-Salmon分期法(D-S分期法)和常用肿瘤负荷指标β2微球蛋白(β2-MG)的相关性,探讨miR-16在MM的发生、发展及预后中的意义。方法:收集43例MM患者及20例正常对照的骨髓标本,用D-S分期法对MM患者进行临床分期,行髂穿获取BMMNC,采用实时荧光定量PCR(real-time PCR)的方法检测BMMNC中miR-16表达,同步抽取外周血,检测β2-MG水平。结果:①miR-16在MM中表达水平明显低于正常对照组,复发/难治组MM患者miR-16表达水平明显低于初治组MM患者,差异有统计学意义(P<0.05);②MM患者化疗后miR-16表达水平较化疗前明显升高(P<0.05),其中以化疗有效组升高最为明显(P<0.05),无效/进展组miR-16表达水平化疗前后无明显改变;③miR-16表达水平与患者病程D-S分期和β2-MG水平有相关性)。结论:miR-16低表达在MM的发病中发挥着重要作用,与MM肿瘤细胞的增殖、转移的能力和疾病预后有密切关系;miR-16表达水平与β2-MG、D-S分期呈负相关,可作为判断MM患者预后指标之一。
Objective: To detect the expression of microRNA-16 (miR-16) in bone marrow mononuclear cells (BMMNC) from patients with multiple myeloma (MM) and analyze the relationship between miR-16 expression and Durie-Salmon staging Tumor load index β2 microglobulin (β2-MG), to explore the significance of miR-16 in the occurrence, development and prognosis of MM. Methods: Bone marrow specimens of 43 patients with MM and 20 normal controls were collected. The clinical stage of MM patients was staged by DS staging. The iliac perforation was used to obtain BMMNC. The expression of miR in BMMNC was detected by real-time PCR -16 expression, synchronous extraction of peripheral blood, β2-MG levels were measured. Results: ① The expression level of miR-16 in MM was significantly lower than that in the normal control group. The expression of miR-16 in MM patients with relapsed / refractory group was significantly lower than that in the untreated group (P <0.05); ②MM The level of miR-16 in patients after chemotherapy was significantly higher than that before chemotherapy (P <0.05), especially in the chemotherapy-effective group (P <0.05), and there was no significant change in the expression of miR-16 in the invalid / progression group before and after chemotherapy; (3) The expression of miR-16 was correlated with DS stage and β2-MG level of patients. Conclusion: The low expression of miR-16 plays an important role in the pathogenesis of MM and is closely related to the proliferation and metastasis of MM tumor cells and the prognosis of MM. The expression of miR-16 is negatively correlated with the stage of β2-MG and DS, Can be used as a prognostic indicator of MM patients.
