目的探讨二维斑点追踪成像技术(2D-STI)评价左束支传导阻滞(LBBB)患者左心室(LV)心肌应变的价值。方法采用2D-STI检测20例孤立性LBBB患者[A组,LV射血分数(LVEF)≥50%]、20例合并有心力衰竭的LBBB患者(B组,LVEF<50%)及25例正常人群(C组)LV的整体纵向峰值应变(GLPS)、心外膜GLPS(GLPS-epi)、心内膜GLPS(GLPS-endo)及LV内应变达峰时间的标准差(T-SD)。结果 A、B组GLPS、GLPS-endo和T-SD均差于C组,且B组GLPS-epi差于C组(P<0.01)。A组GLPS、GLPS-epi、GLPS-endo和T-SD均优于B组(P<0.01)。结论 2D-STI可评估LBBB患者LV心肌应变,GLPS-endo是反映病情的敏感指标。
Objective To investigate the value of 2D-STI in assessing left ventricular (LV) myocardial strain in patients with left bundle branch block (LBBB). Methods Twenty cases of isolated LBBB patients [group A, LV ejection fraction (LVEF) ≥50%] were studied by 2D-STI. Twenty patients with LBBB with heart failure (group B, LVEF <50%) and 25 normal The global longitudinal peak strain (GLPS) of LV, GLPS-epi, GLPS-endo, and standard deviation of peak time of LV in the population (group C) were also measured. Results The GLPS, GLPS-endo and T-SD in groups A and B were all worse than those in group C, and the GLPS-epi in group B was worse than that in group C (P <0.01). GLPS, GLPS-epi, GLPS-endo and T-SD in group A were better than those in group B (P <0.01). Conclusion 2D-STI can assess LV myocardial strain in patients with LBBB, and GLPS-endo is a sensitive indicator of the disease.
