目的:探讨围手术期输血对肝恶性肿瘤术后远期复发率及近期并发症的影响。方法:回顾性分析90例肝恶性肿瘤手术患者,按照有无输血分为2组,每组45例,对其临床资料进行分析。结果:1随访90例肝恶性肿瘤手术患者显示35例患者复发,其中输血组有25例(71.4%)患者复发,无输血组有10例(28.6%)复发,输血组患者术后复发显著高于无输血组(P<0.05),多元回归分析显示,TNM分期、淋巴结转移、围手术期输血均为影响患者预后的独立危险因素。2输血组患者肿瘤大小、侵袭度、淋巴结转移、TNM分期与无输血组比较,差异有统计学意义(P<0.05)。结论:肝恶性肿瘤患者术后复发率及相关并发症的发生与围术期输血存在着密切的关系,围手术期输血可作为判断肝恶性肿瘤远期复发的危险因素。
Objective: To investigate the effect of perioperative blood transfusion on postoperative long-term recurrence and recent complications of liver malignant tumor. Methods: Ninety patients with liver cancer were retrospectively analyzed. The clinical data were analyzed according to the presence or absence of blood transfusion in two groups, 45 cases in each group. A total of 90 patients with liver cancer were followed up for recurrence in 35 patients. Among them, 25 patients (71.4%) in the transfusion group relapsed, and 10 patients (28.6%) in the transfusion group relapsed. The recurrence rate was significantly higher in the transfusion group Multivariate regression analysis showed that TNM staging, lymph node metastasis and perioperative blood transfusion were both independent risk factors influencing prognosis in patients without blood transfusions (P <0.05). There were significant differences in tumor size, invasiveness, lymph node metastasis and TNM stage between the two groups (P <0.05). Conclusion: The incidence of postoperative recurrence and related complications in patients with liver cancer is closely related to perioperative blood transfusion. Perioperative blood transfusion can be used as a risk factor for long-term recurrence of hepatic malignancies.
