目的:评价海藻酸钠-聚赖氨酸-海藻酸钠(APA)微囊化牛肾上腺嗜铬细胞BCC移植于患者蛛网膜下腔后的不良反应。()方法:采用腰椎穿刺术,将微胶囊按0.5×107,1.0×107,1.25×107,1.5×107/次·人4个剂量组,注入癌痛患者蛛网膜下腔内。结果:全组发生I,II度或轻度不良反应14例70%),其中腰腿酸痛((65%)、头晕(50%)、头痛(35%)、发热(25%)、乏力(30%)、恶心(5%)、心悸(5%),持续中位时间在0.5×107,1.0×107剂量组多为两三天,1.25×107,1.5×107剂量组多为4~11d;发生重度不良反应2例(10%),均为1.5×107剂量组,分别表现为双下肢频发抽搐、肩背部压迫感,持续2~5d,无需特殊处理可自愈。移植后患者的生命体征、肝肾功能、免疫功能均无明显改变。结论:APA-BCCs微胶囊蛛网膜下腔移植具有操作简便、不良反应轻、安全的特点,尤其剂量低于1.5×107/次·人更为安全。
OBJECTIVE: To evaluate the adverse reactions of alginate-polylysine-alginate (APA) microencapsulated Bovine adrenal chromaffin cells transplanted into the subarachnoid space of patients. Methods: The lumbar puncture was used to inject the microcapsules into the subarachnoid space of patients with cancer pain in the doses of 0.5 × 107, 1.0 × 107, 1.25 × 107 and 1.5 × 107 / Results: 14 cases (70%) had I, II degree or mild adverse reactions in the whole group, including waist and leg pain (65%), dizziness (50%), headache (35%), fever (25% 30%), nausea (5%), palpitations (5%), the median sustained duration was 0.5 × 107, 1.0 × 107 dose group for more than two days, 1.25 × 107,1.5 × 107 dose group for 4 ~ 11d ; Severe adverse reactions occurred in 2 cases (10%), both 1.5 × 107 dose group, were characterized by frequent lower extremity convulsions, shoulder and back pressure, continued 2 ~ 5d, without special treatment can be self-healing. Vital signs, liver and kidney function, immune function had no significant change.Conclusion: APA-BCCs microcapsule subarachnoid transplantation with simple operation, adverse reactions, light, safe, especially the dose of less than 1.5 × 107 / For safety.
