一、病历摘要:患者女性,57岁,10多年前有过高血压史。1979年6月12日住院。患者两年来感心前区闷胀不适,时有心悸或阵阵眩晕,夜间喜高枕卧位,一度有过双下肢及全身浮肿,5天前受凉后有畏寒发热、头痛,心悸加重。检查:一般情况尚可,半坐位可见颈静脉明显充盈,胸廓无畸形,双肺底可闻及细湿罗音,心尖搏动弥散,心界向左下扩大,心率50次/分。节律不齐,心尖区可听到第四心音及收缩期Ⅲ级吹风样杂
