目的 对老年性白内障患者在白内障超声乳化手术中晶状体后囊膜破裂的原因进行分析 ,总结术中采用的处理方法。方法 对眼科过去 4年来 ,手术中后囊膜破裂 2 0例经妥善处理后进行分析发现 ,术中能及时发现破裂 ,利用粘弹剂的作用 ,均能顺利植入人工晶体。结果 晶状体后囊膜破裂的发生时间 :超声乳化晶状核块时 1 0只眼 (50 % ) ,超声乳化晶状体核上皮质壳时 6只眼 (30 % ) ,注吸晶状体皮质时 4只眼 (2 0 % )。 1 8只眼植入后房型人工晶状体 ,其中晶状体囊袋内植入 1 2例 ,睫状沟植入 6例。 2只眼植入吊线型人工晶状体。术后矫正视力 :为 0 .1—0 .3者 2只眼 (1 0 % ) ,0 4— 0 .8者 9只眼 (45 % ) ,0 .9者 9只眼 (45 % )。结论 在白内障超声乳化中 ,及时发现后囊膜破裂 ,并妥善处理 ,仍能顺利植入人工晶体 ,获取良好术后视力
Objective To analyze the causes of posterior capsular rupture in phacoemulsification cataract patients with senile cataract and to summarize the treatment methods adopted in this study. Methods For the past 4 years in ophthalmology, 20 cases of postoperative rupture of posterior capsule had been properly treated and analyzed. It was found that intraoperative rupture could be found in time and the intraocular lens could be successfully implanted with the use of viscoelastic agent. Results Occurrence of posterior capsular rupture: 10 eyes (50%) in phacoemulsification, 6 eyes (30%) in phacoemulsification and 4 eyes in lens cortex (20%). 18 eyes were implanted posterior chamber intraocular lens, of which 12 cases were implanted in the capsular bag and 6 cases were implanted in the ciliary sulcus. Two eyes implanted suspended intraocular lens. Postoperative corrected visual acuity: 2 eyes (0%) with 0.1-1-0.3 eyes, 9 eyes (45%) with 0-4.0.8 eyes, and 9 eyes (45%) with 0.9 eyes. Conclusions In cataract phacoemulsification, posterior capsular rupture can be found in time and properly treated. Intraocular lens (IOL) can still be successfully implanted to obtain good postoperative visual acuity
