目的 探讨糖尿病微血管病变患者血清瘦素水平变化规律。方法 对照组、糖尿病无微血管病变组及糖尿病微血管病变组各 3 0例 ,其中糖尿病微血管病变组包括糖尿病肾病 2 5例 ,糖尿病视网膜病变 12例 ,同时合并DN与DR者 7例。检测血清瘦素、胰岛素、Ⅳ型胶原及血糖浓度 ,分析瘦素水平与其它指标的相关性。结果 对照组、无微血管病变组及微血管病变组血清瘦素浓度分别为 ( 7.2 0± 2 .11) μg/L、( 7.95± 3 .78) μg/L及 ( 10 .2 6± 4.3 7) μg/L ,糖尿病微血管病变组显著高于无微血管病变组 ( t =2 .18,P <0 .0 5 )及对照组 ( t=2 .71,P <0 .0 1) ,而后两者间无显著差异 ;瘦素浓度与体重指数、胰岛素呈正相关 (r =0 .2 9,P <0 .0 5 ;r =0 .3 4,P <0 .0 1)。结论 糖尿病微血管病变血清瘦素浓度升高 ,瘦素与糖尿病微血管病变的发病密切相关
Objective To investigate the changes of serum leptin level in patients with diabetic microangiopathy. Methods The control group, diabetic microangiopathy group and diabetic microangiopathy group had 30 cases each. Diabetic microangiopathy group included 25 cases of diabetic nephropathy, 12 cases of diabetic retinopathy and 7 cases of DN and DR. Serum leptin, insulin, type Ⅳ collagen and blood glucose levels were measured, and the correlation between leptin level and other indexes was analyzed. Results The serum levels of leptin in control group, non-microvascular group and microvascular group were (7.2 ± 2.11) μg / L, (7.95 ± 3.78) μg / L and (10.26 ± 4.3 7) μg / L, diabetic microangiopathy group was significantly higher than those without microvascular disease (t = 2.18, P <0.05) and control group (t = 2.71, P <0.01) There was no significant difference between the two groups. Leptin concentration was positively correlated with body mass index and insulin (r = 0.29, P <0.05; r = 0.34, P <0.01). Conclusions The concentration of serum leptin in diabetic microangiopathy is increased. Leptin is closely related to the pathogenesis of diabetic microangiopathy
