目的:比较计算机辅助细胞学检测(computer-assisted cytological test,CCT)、液基细胞学检查(liquid-basedcytological test,LCT)、LCT联合二代杂交捕获系统(hybrid captureⅡ,HC2)HPV DNA检测在宫颈癌筛查中的灵敏度和特异度。方法:CCT检测的全部病例与LCT检测的部分病例为回顾性病例。HPV DNA检测选用HC2法,剩余标本继续做LCT检测。结果:HC2 HPV DNA结合LCT检查的敏感性显著高于CCT及LCT两种检查方法检查的敏感性(P<0.05)。CCT检测宫颈癌的特异性是88.14%,LCT的特异性是93.78%,LCT联合HC2 HPV DNA检测的特异性是100.00%。三者特异性比较差异均具有统计学意义(P<0.05)。结论:CCT和LCT是目前有效的宫颈癌筛查手段。HC2 HPV DNA检查结合LCT是筛查宫颈癌及癌前病变最好的方法,其阴性预测值几乎达到100%。
Objective: To compare the diagnostic value of HPV DNA detection in computer-assisted cytological test (CCT), liquid-based cytological test (LCT), LCT and hybrid captureⅡ (HC2) Sensitivity and specificity in cancer screening. Methods: All the cases detected by CCT and some cases detected by LCT were retrospective cases. HC2 DNA detection method used HC2, the remaining specimens continue to do LCT test. Results: The sensitivity of HC2 HPV DNA in combination with LCT was significantly higher than that of both CCT and LCT (P <0.05). The specificity of CCT for detecting cervical cancer was 88.14%, the specificity of LCT was 93.78%, and the specificity of LCT combined with HC2 HPV DNA test was 100.00%. The specificity of the three differences were statistically significant (P <0.05). Conclusion: CCT and LCT are the effective screening methods for cervical cancer. HC2 HPV DNA test combined with LCT is the best screening method for cervical cancer and precancerous lesions, the negative predictive value of almost 100%.
