目的探讨正常高值血压人群左心房、左心室结构及QT离散度的改变。方法选择在本院体检中心行健康体检中血压<140/90mmHg的人群为研究对象,依据JNC7将研究对象分为正常血压组:<120/80mmHg,正常高值血压组:120-139/80-89mmHg。全部研究对象均测空腹血糖、血脂、血钾、血肌酐浓度,测定尿白蛋白和肌酐浓度,计算尿白蛋白/肌酐比值(ACR)等指标,测体重指数(BMI)、腰围和臀围。行心电图及超声心动图检查。结果⑴两组间比较,年龄、BMI、腰围、臀围、腰围/臀围、空腹血糖、血脂、血钾、血肌酐浓度、ACR的差异无统计学意义。⑵与正常血压组人群比较,正常高值血压组人群的左心房内径、收缩期末室间隔厚度及收缩期末左心室内径均明显增加,差异有统计学意义;最小QT(QTmin)及最小QTc(QTcmin)均明显缩小,差异有统计学意义;QT离散度(QTd)及校正QT离散度(QTcd)明显增大,且有统计学意义。结论正常高值血压人群可以发生左心室、左心房结构的改变及QTd的异常。
Objective To investigate the change of left atrium, left ventricular structure and QT dispersion in normal high blood pressure population. Methods The subjects with blood pressure <140 / 90mmHg in the physical examination of our hospital were selected as the study subjects. According to JNC7, the subjects were divided into two groups: normal blood pressure group (120 / 80mmHg), normal high blood pressure group (120-139 / 80- 89mmHg. All subjects were measured fasting blood glucose, serum lipids, serum potassium, serum creatinine concentrations, urinary albumin and creatinine concentrations were calculated urinary albumin / creatinine ratio (ACR) and other indicators measured body mass index (BMI), waist circumference and hip circumference. Compared with normal blood pressure group, left atrial diameter, systolic ventricular septal thickness and systolic left ventricular diameter were significantly increased in normal high blood pressure group, the difference was statistically significant; the minimum QT (QTmin) and QTcmin ) Were significantly reduced, the difference was statistically significant; QTd and QTd dispersion (QTcd) significantly increased, and with statistical significance. Conclusion The changes of left ventricular and left atrial structure and QTd abnormalities may occur in normal high blood pressure population.
