目的了解延边地区朝鲜族和汉族学生脂肪细胞因子和不同代谢指标水平及两者的关联性,为早期防控儿童代谢综合征提供参考。方法分别于2011年7月15—18日和2013年7月16—21日,共抽取5 747名延边地区小学生(朝鲜族2 011名,汉族3 736名)作为研究对象,测量调查对象身高、体重、腰围和血压,并测量清晨空腹血清三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、血糖(FBG)、胰岛素(INS)、脂联素、瘦素和脂肪细胞脂肪酸结合蛋白(A-FABP)。结果调整年龄和性别的协方差分析结果显示,延边地区朝鲜族学生脂联素、瘦素和A-FABP平均水平分别为(1.84±0.71)(1.23±0.27)和(4.39±0.67)ng/mL,汉族学生分别为(2.02±0.44)(1.15±0.18)和(4.25±0.96)ng/mL,差异均有统计学意义(P值均<0.01);延边地区朝鲜族学生体质量指数(BMI)、腰围(WC)、收缩压(SBP)、舒张压(DBP)、三酰甘油(TG)、血糖(FBG)和稳态胰岛素评价指数(HOMA-IR)平均水平均高于汉族学生,而HDL-C平均水平低于汉族学生(P值均<0.01)。方差分析结果显示,不同脂联素、瘦素和A-FABP水平朝鲜族和汉族学生代谢指标分布特征差异有统计学意义。多元线性回归分析结果显示,学生脂联素与年龄、性别、民族、BMI,WC和HOMA-IR均密切相关,瘦素与年龄、性别、民族、BMI,WC,SBP,DBP,TG,HDL-C和HOMA-IR均密切相关(P值均<0.05);A-FABP与民族、BMI,WC,TG和HOMA-IR均密切相关(P值均<0.05)。结论朝鲜族和汉族学生脂肪细胞因子和不同代谢指标的分布存在差异;脂肪细胞因子与不同代谢指标的相关性程度有所不同,且存在民族差异。
Objective To understand the association of adipocytokines and different metabolic indexes of Korean and Han nationality students in Yanbian area and to provide a reference for early prevention and control of children’s metabolic syndrome. Methods A total of 5 747 primary school students in Yanbian (2 011 from Korea and 3 736 Han from Han nationality) were sampled from July 15-18, 2011 and July 16-21, 2013 respectively. Subjects were surveyed to measure the height, Body weight, waist circumference and blood pressure. Fasting serum triglyceride (TG), high density lipoprotein cholesterol (HDL-C), blood glucose (FBG), insulin (INS), adiponectin, leptin and adipocyte fatty acid Protein (A-FABP). Results The results of covariance analysis adjusted age and gender showed that the average levels of adiponectin, leptin and A-FABP in Korean students in Yanbian were (1.84 ± 0.71) (1.23 ± 0.27) and (4.39 ± 0.67) ng / mL , Han students (2.02 ± 0.44) (1.15 ± 0.18) and (4.25 ± 0.96) ng / mL, respectively (all P <0.01). The body mass index (BMI) WC, SBP, DBP, triglyceride (TG), blood glucose (FBG) and homeostasis insulin assessment index (HOMA-IR) were higher than those of Han students, but HDL The average level of C was lower than Han students (P <0.01). Analysis of variance showed that there were significant differences in the distribution of metabolic parameters among Korean and Han students with different levels of adiponectin, leptin and A-FABP. Multivariate linear regression analysis showed that adiponectin was closely related to age, sex, ethnicity, BMI, WC and HOMA-IR. Leptin was positively correlated with age, gender, ethnicity, BMI, WC, SBP, DBP, TG, HDL- C and HOMA-IR were closely related (P <0.05). A-FABP was closely related to nationality, BMI, WC, TG and HOMA-IR (all P values <0.05). Conclusion There are differences in the distribution of adipocytokines and different metabolic indexes among Korean and Han students. The correlation between adipocytokines and different metabolic indexes is different, and there are national differences.
