目的:探讨恒牙早期男性青少年上气道矢状向空间大小与颅颌面骨结构的相关性,从而找出影响气道大小的骨性特征。方法:收集74例恒牙早期无鼾男性错畸形患者(年龄11~16岁,平均年龄13岁)治疗前头颅侧位片,对颅颌面骨结构及上气道矢状向空间大小进行Delaire头影测量分析。应用SPSS13.0软件包对数据进行处理,采用多元回归分析法研究上气道矢状面大小与颅颌面骨结构异常程度之间的关系。结果:鼻咽气道矢状径(PNS-ad)与面上份高度(UFH)、下颌支后缘的倾斜度(RA)以及颅高与颅底长的比值(Ht/Dp)呈负相关,与前颅底角(CFA)呈正相关;软腭后气道最小矢状径(Ve-PVe)与面上份高度(UFH)、颅高与颅底长的比值(Ht/Dp)呈负相关;舌根后气道最小矢状径(Ph1-Ph2)与颈椎和颅底夹角(CVTA)呈正相关,与下颌平面角(MPA)、下颌体长度(MnBL)、颅高与颅底长的比值(Ht/Dp)呈负相关。结论:恒牙早期无鼾男性错患者上气道矢状向空间大小与颅颌面骨以及头颈姿势关系密切,颅面生长型和气道周围邻近骨结构对气道影响较大。
Objective: To explore the correlation between the sagittal space size of upper airway and the structure of craniomaxillofacial in early adolescent young permanent children, so as to find out the skeletal features that affect the airway size. Methods: A total of 74 patients with craniofacial bone structure and upper sagittal space size were collected from 74 patients with early non-snoring male malocclusion (aged 11-16 years, mean age 13 years) before treatment Delaire Cephalometric Analysis. SPSS 13.0 software package was used to process the data and multivariate regression analysis was used to study the relationship between the size of the upper airway sagittal plane and the degree of abnormal cranial and maxillofacial structure. Results: PNS-ad was negatively correlated with UFH, RA of the mandibular branch, and the ratio of skull height to skull base length (Ht / Dp) (P <0.05). There was a positive correlation between Ve-PVe and UFH and the ratio of skull base height (Ht / Dp) to the anterior skull base angle (CFA) The mean sagittal diameter (Ph1-Ph2) of the posterior lingual base was positively correlated with the angle of the cervical vertebra and the base of the skull (CVTA). The ratio of mandibular plane angle (MPA), mandibular body length Ht / Dp) was negatively correlated. Conclusion: The size of sagittal space of upper airway is closely related to craniomaxillofacial and head-and-neck posture in patients with early non-snoring male malocclusion. Craniofacial growth and adjacent bone structures around airway have a greater influence on airway.
