诊断卡培他滨治疗胃癌严重手足综合征1例,表现为严重手足的皮肤改变,出现脱皮、流血、起疱、水肿,疼痛,自理能力受限。采用多种途径补充水电解质,静脉补充氯化钠等水电解质。给予抗真菌、抗感染治疗。静脉注射丙种免疫球蛋白,1次,0.5g/kg。加强皮肤护理,口服维生素B6,400mg/d。My5-FU进行血浆检测,结果为245.4ng/ml,安排血药滤过治疗,2日后下降为88ng/ml,次日低于检测范围。皮肤15日愈合,未见感染等严重并发症。
