目的探讨轻度认知障碍(mild cognitive impairment,MCI)患者的脑血管反应性(CVR)改变。方法根据蒙特利尔量表(Mo CA)的评估结果,将136例疗养员分为MCI组(68例)和对照组(68例),收集临床资料并分析认知损害特点;采用经颅多普勒(TCD)屏气试验,检测两组人群的屏气指数(BHI),作为CVR的评价指标。结果 MCI组空腹血糖(FBG)、总胆固醇(TG)、三酰甘油(TC)、收缩压(SBP)均高于对照组(P<0.05);MCI组除了总体认知水平降低,各认知域中视空间及执行功能、注意、抽象、延迟回忆功能明显受损(P<0.05);BHI值明显低于对照组(P<0.05)。结论 MCI的发生与血糖、血脂、血压控制不佳相关,认知损害以视空间及执行功能、注意、抽象、延迟回忆功能为主。MCI患者的脑血管反应性受损。
Objective To investigate the changes of cerebrovascular reactivity (CVR) in patients with mild cognitive impairment (MCI). Methods According to the assessment results of Mo CA, 136 nursing staffs were divided into MCI group (68 cases) and control group (68 cases). The clinical data were collected and the characteristics of cognitive impairment were analyzed. The transcranial Doppler (TCD) breath test. The breath holding index (BHI) of the two groups was measured as the evaluation index of CVR. Results The fasting blood glucose (FBG), total cholesterol (TG), triglyceride (TC) and systolic blood pressure (SBP) in MCI group were significantly higher than those in control group (P <0.05) (P <0.05). The BHI was significantly lower than that of the control group (P <0.05). Conclusion The occurrence of MCI is related to the poor control of blood glucose, blood lipids and blood pressure. Cognitive impairment mainly depends on visual space, executive function, attention, abstraction and delayed memory. Cerebral vascular reactivity is impaired in MCI patients.
