目的 探讨54 例以植物神经功能障碍(AD)为首发症状的Guillain-Barre综合征(GBS)患者的临床特征、早期诊断、转归及预防措施。方法 对54 例GBS患者的临床资料进行分析。结果 发现其临床特征与一般GBS有所不同:(1)AD表现以手足肿胀最多,恶心呕吐次之,胸闷心悸较少;(2)合并颅神经损害多(53.7% );(3)以肢体远端肌无力明显(76.5% );(4)感觉障碍出现率高(83.3% );(5)脑脊液呈现蛋白-细胞分离现象少(10.2% );(6)预后较良好。结论 以AD为首发症状的GBS临床上不少见,早期误诊率高,应引起临床高度重视。
Objective To investigate the clinical features, early diagnosis, prognosis and preventive measures of 54 cases of Guillain-Barre syndrome (GBS) with autonomic dysfunction (AD) as the first symptom. Methods The clinical data of 54 patients with GBS were analyzed. The results showed that the clinical features were different from the general GBS: (1) AD manifested the most swelling of hands and feet, followed by nausea and vomiting, followed by less chest tightness and palpitations; (2) more cranial nerve damage (53.7%); (3) (76.5%); (4) high incidence of sensory disturbances (83.3%); (5) cerebrospinal fluid showed less protein-cell detachment (10.2%); (6) The prognosis is good. Conclusions The GBS with AD as the first symptom is clinically not uncommon, with a high rate of misdiagnosis in the early stage and should be highly valued in clinical practice.
