目的探讨血液透析患者外周血白细胞分类与透析中低血压及心血管事件的预测价值。方法记录患者白细胞分类,透析中低血压及心血管事件的发生,探讨白细胞分类、透析中低血压与心血管事件的关系。结果观察组患者白细胞计数、中性粒细胞比例、单核细胞比例、中性粒细胞/淋巴细胞(N/L)比值、透析中低血压比例均高于对照组患者,淋巴细胞比例比对照组患者低,差异具有统计学意义(P<0.05)。COX回归分析结果提示N/L上升0.1(P=0.014)、透析中低血压(P=0.04)、中性粒细胞比例上升10%(P=0.012)是患者心血管事件的独立风险因素。结论透析中低血压及中性粒细胞/淋巴细胞比均是血液透析患者心血管事件的独立风险因素。
Objective To investigate the classification of peripheral leukocytes in hemodialysis patients and the predictive value of hypotension and cardiovascular events during dialysis. Methods The classification of white blood cells, the incidence of hypotension and cardiovascular events in dialysis patients were recorded. The classification of leucocytes, the relationship between hypotension and cardiovascular events in dialysis were analyzed. Results The white blood cell count, neutrophil ratio, monocyte ratio, neutrophil / lymphocyte ratio (N / L) ratio and dialysis middling hypotension rate in the observation group were higher than those in the control group. The proportion of lymphocytes in the observation group was higher than that in the control group Patients with low, the difference was statistically significant (P <0.05). COX regression analysis showed that N / L increased by 0.1 (P = 0.014), moderate hypotension (P = 0.04) and neutrophil increased by 10% (P = 0.012) were independent risk factors for cardiovascular events. Conclusions Both dialysis-induced hypotension and neutrophil / lymphocyte ratio are independent risk factors for cardiovascular events in hemodialysis patients.
