目的探讨动态脑电图(AEEG)在儿童癫痫中的诊断价值。方法 2007年9月至2011年9月,对濮阳市妇幼保健院收治的100例癫痫或者疑似癫痫患儿的临床资料进行回顾性分析。结果 100例癫痫或者疑似癫痫患儿,有52例监测到痫样放电,占52.0%;有16例脑电图监测到非特异性异常,占16.0%;32例脑电图未见异常,占32.0%。监测中可出现1~2次的临床发作,少数持续时间可达15 min左右。动态脑电图痫样放电52例中,棘波3例,棘慢波1例,多棘慢波1例,尖波18例,尖慢波29例。清醒期监测到痫样放电9例,睡眠期监测到痫样放电33例,清醒和睡眠期均出现痫样放电10例。结论动态脑电图能够检测清醒期和睡眠各期的脑电波形,是诊断儿童癫痫的主要辅助检查方法,能为临床诊断提供可靠的依据。
Objective To investigate the diagnostic value of dynamic electroencephalogram (AEEG) in children with epilepsy. Methods From September 2007 to September 2011, the clinical data of 100 cases of epilepsy or suspected epilepsy admitted to Puyang Maternal and Child Health Hospital were analyzed retrospectively. Results 100 cases of epilepsy or suspected epilepsy, 52 cases were detected epileptiform discharges, accounting for 52.0%; 16 cases of EEG non-specific abnormalities were monitored, accounting for 16.0%; 32 cases of EEG no abnormalities, accounting for 32.0 %. Monitoring can occur 1 to 2 times the clinical attack, a few durations up to 15 min or so. In 52 cases of dynamic EEG epileptiform discharge, there were 3 spikes, 1 spike and 1 spike, 1 spike and 1 spike, 18 spikes and 29 spikes. Nine cases of epileptiform discharge were monitored in awake period, 33 cases of epileptiform discharge were observed in sleep period, and 10 cases of epileptiform discharge in awake and sleep stage. Conclusion The dynamic EEG can detect the EEG waveform during the awake and sleep stages. It is the main auxiliary examination method for diagnosing epilepsy in children, which can provide a reliable basis for clinical diagnosis.
