本文采用放射免疫法测定42例无临床蛋白尿的Ⅱ型糖尿病患者和20例健康人血小板膜表面α-颗粒膜蛋白(GMP-140)分子数和24 h 尿白蛋白定量。结果是①糖尿病组 GMP-140、24h 尿白蛋白排泄(UAE)均明显高于正常对照组(P<0.01);②糖尿病组中,76.2%的患者 GMP-140升高,61.9%的患者 UAE 升高,二者差异显著(P<0.01),GMP-140与 UAE 呈正相关(r=0.91,P<0.01);③在 UAE 升高的糖尿病患者中,96.2%患者 GMP-140升高,而 GMP-140升高的糖尿病患者中,68.8%患者 UAE 升高;④与无视网膜病变组相比,有视网膜病变组 GMP-140、UAE 升高的阳性率增加(P<0.05~0.01)。提示 GMP-140同样可作为Ⅱ型糖尿病肾病早期诊断指标,甚至较 UAE 敏感。
In this paper, 42 cases of type 2 diabetes mellitus without clinical proteinuria and 20 healthy people were detected by radioimmunoassay and the 24-hour urinary albumin was quantified. The results were as follows: ① The levels of urinary albumin excretion (GFA-140, 24h) in diabetic group were significantly higher than those in normal control group (P <0.01); ② In GDM group, GMP-140 increased in 76.2% of diabetic patients and UAE (P <0.01). There was a positive correlation between GMP-140 and UAE (r = 0.91, P <0.01). (3) Among the UAE patients with elevated UAE, GMP-140 increased in 96.2% UAE increased in 68.8% patients with elevated GMP-140. (4) Compared with those without retinopathy, the positive rate of GMP-140 and UAE increased in patients with retinopathy (P <0.05-0.01). Tip GMP-140 can also be used as an early diagnosis of type II diabetic nephropathy index, even more sensitive than UAE.
