目的分析右丙亚胺与参麦注射液联合治疗对减少急性白血病化疗中心脏毒性的影响。方法选取2012年3月-2015年5月就诊于淄博市中心医院的急性白血病患者56例,随机分为对照组和研究组,每组28例,对照组给予右丙亚胺治疗,研究组给予右丙亚胺联合参麦注射液治疗。观察两组患者的心肌酶数值变化,包括磷酸肌酸激酶(CK)、心肌型肌酸激酶同功酶(CKMB)、乳酸脱氢酶(LDH)、α-羟丁酸脱氢酶(α-HBDH)、谷丙转氨酶(AST),心电图改变及治疗前后左室射血分数(LVEF)、心肌肌钙蛋白T(c Tn T)、B型利钠肽(BNP)水平变化。结果化疗后研究组患者的CK、CKMB、LDH、α-HBDH水平均明显低于对照组(P<0.05);研究组患者的心电图异常率为32.14%(9/28),低于对照组的64.29%(18/28)(P<0.05);化疗后两组患者的LVEF水平均较化疗前降低,且研究组降低程度明显高于对照组(P<0.05);两组患者的c Tn T、BNP水平均较化疗前升高,且研究组升高程度明显高于对照组(P<0.05)。结论右丙亚胺联合参麦注射液进行急性白血病化疗能够有效降低蒽环类化疗药物的心脏毒性。
Objective To analyze the effect of combination of dexrazoxane and shenmai injection on the reduction of cardiotoxicity in acute leukemia chemotherapy. Methods From March 2012 to May 2015, 56 patients with acute leukemia visited Zibo Central Hospital were randomly divided into control group and study group, with 28 cases in each group. The control group was treated with dexmedetamil and the study group was given Right propylamine combined with shenmai injection. The changes of myocardial enzymes were observed in two groups, including CK, CKMB, LDH and a-hydroxybutyric acid dehydrogenase (α- (HBDH), alanine aminotransferase (AST), changes of electrocardiogram (ECG) and left ventricular ejection fraction (LVEF), cardiac troponin T (cTn T) and B-type natriuretic peptide (BNP) Results The levels of CK, CKMB, LDH and α-HBDH in the study group after chemotherapy were significantly lower than those in the control group (P <0.05). The abnormality rate of ECG in the study group was 32.14% (9/28) 64.29% (18/28) (P <0.05). The levels of LVEF in both groups after chemotherapy were lower than those before chemotherapy, and the reduction in the study group was significantly higher than that in the control group (P <0.05). The cTn T , BNP levels were higher than before chemotherapy, and the study group was significantly higher than the control group (P <0.05). Conclusion Combination of dexrazoxane and shenmai injection for acute leukemia chemotherapy can effectively reduce the cardiotoxicity of anthracycline chemotherapy.
