目的应用速度向量成像技术(VVI)和常规超声心动图评价美沙酮维持治疗者左心室收缩功能,以探索早期反映心肌收缩功能异常的敏感性的指标。方法 30例健康者及30例美沙酮维持治疗者,取心尖四腔的速度向量图像,测定左室各节段的收缩期最大速度(Vp)、位移(D)、应变(S)及应变率(SR)。结果①超声心动图结果与对照组相比,处理组的左室整体功能无明显变化(P>0.05)。②VVI测量结果与对照组健康者相比,处理组美沙酮维持治疗者的左心室各节段的最大位移、速度均有所下降,应变、应变率显著下降(P<0.05),说明海洛因依赖者左室局部功能较正常人显著下降。结论与常规测量左室功能的方法相比,速度向量成像技术能够客观、准确地评价海洛因依赖者的心肌运动功能,对临床诊治及判断预后具有重要的指导意义。
Objective To evaluate left ventricular systolic function in methadone maintenance therapy using velocity vector imaging (VVI) and conventional echocardiography to explore the early indicators that reflect the sensitivity of myocardial contractile dysfunction. Methods The velocity vector images of the atrium and quadruple ventricle were obtained from 30 healthy subjects and 30 patients with methadone maintenance. The maximum systolic velocity (Vp), displacement (D), strain (S) and strain rate SR). Results ① echocardiography results compared with the control group, the treatment group had no significant change in left ventricular function (P> 0.05). VVI measurement results showed that the maximal displacement and velocity of the left ventricular segments of the treatment group were decreased, the strain and strain rate decreased significantly (P <0.05) compared with those of the control group, indicating that the left Local function decreased significantly compared with normal subjects. Conclusion Compared with routine measurement of left ventricular function, velocity vector imaging can objectively and accurately evaluate the myocardial function of heroin addicts and has important guiding significance for clinical diagnosis and prognosis.
