目的 探讨实时三维超声心动图 (RT 3DE)在房间隔缺损封堵术中的应用价值。方法 术前经二维超声心动图及RT 3DE检查明确诊断 ,术中全程引导临床 8例房间隔缺损 (ASD)介入治疗手术 ,术后经RT 3DE检查证实封堵效果。结果 8例介入手术均获成功 ,术后无残余分流、心律失常、心包填塞、封堵器脱落、栓塞等并发症。结论 RT 3DE可对缺损的部位、分型、直径、周长、面积做出准确定量诊断 ,无需经食管超声 (TEE)和球囊测径 ,可靠性强 ,且远离放射线。对封堵伞与缺损及心脏内部结构之间的精确定位及操作引导具有二维超声、经食管超声和放射线不可比拟的优势。
Objective To investigate the value of real-time three-dimensional echocardiography (RT 3DE) in occlusion of atrial septal defect. Methods Two-dimensional echocardiography and RT-3DE were used to diagnose preoperatively. Eight patients with atrial septal defects (ASD) underwent interventional surgery during the entire procedure. Postoperative occlusion was confirmed by RT 3DE. Results All the 8 cases were successful in the interventional procedure. No postoperative residual shunt, arrhythmia, pericardial tamponade, occluder shedding and embolization were found. Conclusion RT 3DE can accurately diagnose the location, type, diameter, perimeter and area of the defect without the need of transesophageal echocardiography (TEE) and balloon diameter measurement. It is reliable and far away from radiation. The accurate positioning and operation guidance between the occluded umbrella and the defect and the internal structure of the heart have the advantages of two-dimensional ultrasound, trans-esophageal ultrasound and radiography.
