目的:探讨柚皮苷对去势大鼠骨质疏松性骨折愈合过程的影响。方法:3月龄雌性Wistar大鼠50只,随机分成去势组(OVX)40只,假手术组(SHAM)10只。去势组采用背部双侧去卵巢术建立去势大鼠骨质疏松模型;3个月后,将去势组及假手术组大鼠均造成左侧股骨中点骨折(OPF),然后用克氏针髓内固定,建立骨质疏松性骨折模型。再将制成骨折模型的去势组随机分成模型空白组(OVX+N)和3个实验组(OVX+L/M/H),每组10只,3个实验组分别予以柚皮苷(低剂量组1 mg/kg,中剂量组10 mg/kg,高剂量组100 mg/kg)灌服,SHAM组和OVX+N组给予同体积的双蒸水灌服,共4周。于OPF术后当日、4周、8周,C型臂X光机行X片检查,观察髓内针位置和骨折断端愈合情况;OPF术后8周,处死大鼠,取骨折处骨痂组织,行光镜下组织学观察、血生化指标检测及三点弯曲实验,测量骨痂最大承载负荷。结果:1去势组骨密度值显著低于SHAM组(P<0.05),骨质疏松模型建立。2组织学观察结果显示,术后8周SHAM组见大量成熟骨小梁形成;OVX+M组原始骨小梁逐渐改建成为成熟骨小梁;OVX+L、M两组有大量原始骨小梁,OVX组开始出现少量原始小梁状骨。3实验组中、高剂量组的碱性磷酸酶(B-ALP)、Ⅰ型前胶原羧基端肽(PICP)、骨钙素(BGP)水平较模型组明显降低(P<0.05,P<0.01);低、中、高剂量组之间差异均有统计学意义(P<0.05,P<0.01)。4放射学观察结果示,OPF术后4周X线检测结果示各组均有骨痂形成,SHAM组有大量骨痂,OVX+M、H组有少量骨痂,OVX+N及OVX+L组有较少骨痂。OPF术后8周X线检查示,SHAM组骨折线模糊,有连续骨痂通过骨折断端;OVX+H、M、L三组骨折线稍显模糊,生成骨痂大小依次为H组>M组>L组;OVX+N组骨折线仍清晰。5术后8周生物力学测定示,OVX+N组的三点弯曲载荷明显低于SHAM组(P<0.01),实验组中、高剂量组的载荷较OVX+N组明显增高(P<0.05,P<0.01);低、中、高剂量组之间差异均有统计学意义(P<0.05,P<0.01)。结论:柚皮苷对去卵巢大鼠OPF有促进骨折愈合的作用,且可能存在一定的剂量依赖关系。
Objective: To investigate the effect of naringin on osteoporotic fracture healing in ovariectomized rats. Methods: Fifty female Wistar rats of 3 months old were randomly divided into 40 sham-operated group (OVX) and 10 sham-operation group (SHAM). Ovariectomized rats were used to establish osteoporosis model of ovariectomized rats. Three months later, left femur fracture (OPF) was induced in both castrated and sham-operated rats, ’S intramedullary nailing, the establishment of osteoporotic fracture model. Then the castrated fracture model was randomly divided into model blank group (OVX + N) and three experimental groups (OVX + L / M / H), 10 in each group. The three experimental groups were treated with naringin Low-dose group 1 mg / kg, medium-dose group 10 mg / kg, high-dose group 100 mg / kg) were given orally, SHAM group and OVX + N group were given the same volume of double distilled water for 4 weeks. On the day after OPF, C-arm X-ray was performed on the C-arm X-ray machine to observe the position of the intramedullary nail and the healing of fracture. Eight weeks after OPF, the rats were sacrificed and the callus Tissue, line light microscopy histological observation, blood biochemical markers and three-point bending test to measure the maximum load bearing callus. Results: 1 The BMD of castrated group was significantly lower than that of SHAM group (P <0.05). The osteoporosis model was established. 2 The results of histological observation showed that a large number of mature trabecular bone was formed in SHAM group at 8 weeks after operation; the original trabecular meshwork was gradually transformed into mature trabecular in OVX + M group; , The OVX group began to appear a small amount of the original trabecular bone. Compared with the model group, the level of alkaline phosphatase (B-ALP), type Ⅰ procollagen carboxyterminal peptide (PICP) and BGP in experimental group were significantly lower than those in model group (P <0.05, P <0.01 ). There was significant difference between low, medium and high dose groups (P <0.05, P <0.01). Radiographic observations showed that the X-ray findings at 4 weeks after OPF showed that callus was formed in all groups, a large number of callus was found in SHAM group, and a few callus were found in OVX + M and H groups. OVX + N and OVX + L Group had less callus. Eight weeks after OPF, X-ray examination showed that the fracture line in the SHAM group was fuzzy and the continuous callus passed the broken ends of the fracture. The fracture lines in OVX + H, M and L groups were slightly blurred, and the callus size was H> M Group> L group; the fracture line of OVX + N group was still clear. The biomechanical measurements at 8 weeks postoperatively showed that the three-point bending load of OVX + N group was significantly lower than that of SHAM group (P <0.01), and the load of experimental group was significantly higher than that of OVX + N group , P <0.01). There was significant difference between low, medium and high dose groups (P <0.05, P <0.01). CONCLUSION: Naringin can inhibit the healing of OPF in ovariectomized rats, and there may be some dose-dependent relationship.
