目的探讨人乳头瘤病毒(HPV)在妇女宫颈病变中的型别分布特征及感染特点。方法收集2009~2011年1 213例宫颈病变活检组织标本(CINⅠ317例,CINⅡ233例,CINⅢ208例,宫颈鳞状细胞癌198例,慢性宫颈炎257例),采用PCR-反向点杂交法进行HPV DNA分型检测。结果 HPV在女性宫颈病变中的平均感染率为69.74%,随着病变严重程度的增加HPV感染率亦增加,二者呈正相关(P<0.05)。各组病变中以HPV16的感染率最高,其次为HPV58、18、33、11。CINⅠ组中感染最多的前3位为HPV16、58、11;CINⅡ组中感染最多的前3位为HPV16、58、33;CINⅢ组中感染最多的前3位为HPV16、58、18;SCC组中感染最多的前3位为HPV16、18、58;宫颈炎组中感染最多的前3位为HPV16、58、33。每组病变都存在单纯感染和复合感染,复合感染随着病变严重程度的增加而减少。结论 HPV的感染率及型别分布在不同病变的宫颈组织中均有一定的差异,致癌性肯定的HPV亚型中感染率较高的为HPV16、18和58。
Objective To investigate the type distribution and infection characteristics of human papillomavirus (HPV) in women with cervical lesions. Methods A total of 1 213 cervical biopsy specimens from 2009 to 2011 were collected (CINⅠ317 cases, CINⅡ233 cases, CINⅢ208 cases, cervical squamous cell carcinoma (198 cases) and chronic cervicitis (257 cases)). PCR-reverse dot hybridization was used to detect HPV DNA Typing test. Results The average infection rate of HPV in women with cervical lesions was 69.74%. HPV infection rate also increased with the severity of the disease, and the two were positively correlated (P <0.05). In each group, the infection rate of HPV16 was the highest, followed by HPV58, 18, 33, 11. The first three infected most in CINⅠgroup were HPV16,58,11; the first three most infected in CINⅡgroup were HPV16,58,33; the first three most infected in CINⅢgroup were HPV16,58,18; SCC group Among the top 3 infected most, HPV16, HPV18, HPV18 and HPV58 were the most common infection in cervicitis group. Each group of lesions there is a simple infection and compound infection, the compound infection with the severity of the lesion increased. Conclusion The infection rate and type distribution of HPV in cervical lesions with different pathological changes have certain differences. HPV16, HPV18, and HPV18 are the most prevalent in HPV subtypes.
