男,5岁,车祸伤及胸部4 h。临床表现为浅昏迷和呼吸困难。查体发现患者右侧第2、3前肋骨处触诊有骨擦音,局部皮下软组织肿胀,左肺听诊未闻及呼吸音,叩诊呈实音。胸部正侧位CR片示右侧第2、3前肋骨骨折,左肺不张,右肺下叶结节影考虑肺内血肿;心影增大。建议做CT及心脏彩超检查。
Male, 5 years old, car accident hurt the chest 4 h. Clinical manifestations of shallow coma and breathing difficulties. Physical examination found that the right front 2,3 patients with palpation at the front of the bone fricative, local subcutaneous soft tissue swelling, left lung auscultation and respiratory sounds were not heard, percussion was solid tone. Chest orthopedic CR film showed the first 2, 3 anterior rib fracture, left atelectasis, right lower lobe nodules to consider pulmonary hematoma; heart shadow increased. Recommended to do CT and cardiac color Doppler ultrasound examination.
