对56例早发(DM)及52例(DGTT)及(HbAIC)均正常者(NGT)检测总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、(SUA)和肌酐(SCr),测量体质量指数(BMI)和腰臀比(WHR);用稳态模型胰岛素抵抗指数(HOMA-IR)评估胰岛素抵抗,胰岛β细胞功能指数(HOMA-β)评估岛素基础分泌,采用OGTT中空腹和30分钟胰岛素血糖差值的比值(△I30/△G30)评价胰岛素早期分泌。结果:①DM和NGT组间SUA差异无统计学意义,HDL、HOMA-β、△I30/△G30显著低于NGT组,HOMA-IR、TG、BMI、WHR、OGTT2小时胰岛素显著高于NGT组;②在NGT组,调整年龄、BMI、血压、TG后,男性的尿酸水平仍高于女性;③在所有的个体中,逐步多元回归分析,发现Ser、BMI、性别、TG是影响血尿酸的独立因素。结论:血尿酸水平在2型糖尿病发生的不同阶段无显著性变化,Scr、性别、BMI、TG是影响尿酸水平的独立因素。
The levels of total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), and low density lipoprotein cholesterol (LDL-C) were measured in 56 cases of early-onset and 52 cases of DGTT and normal HbAIC. LDL-C, SCA, and body mass index (BMI) and waist-to-hip ratio (WHR) were measured. Insulin resistance was assessed using homeostasis model insulin resistance index (HOMA-IR) Insulin beta cell function index (HOMA-β) was used to assess insulin basal secretion. Early insulin secretion was evaluated by the ratio of fasting blood glucose (OGTT) and the difference of 30-minute insulin blood glucose (△ I30 / △ G30) Results: ① There was no significant difference in SUA betweenDM and NGT groups, HDL, HOMA-βand △ I30 / △ G30were significantly lower than NGT group. HOMA-IR, TG, BMI, WHR and OGTT2hours insulin was significantly higher than NGT group; ② In the NGT group, the level of uric acid was still higher in men than in women after adjusted for age, BMI, blood pressure and TG; ③ In all individuals, multiple regression analysis showed that Ser, BMI, sex and TG were the independent factors influencing serum uric acid factor. CONCLUSION: Serum uric acid level has no significant change in different stages of type 2 diabetes. Scr, sex, BMI and TG are the independent factors influencing uric acid level.
