目的比较不同治疗措施治疗带状疱疹神经痛的止痛效果,优化带状疱疹神经痛治疗方案。方法通过军卫一号病案管理系统,收集2009年1月1日-2013年12月20日带状疱疹住院患者资料,按治疗措施不同分为5组。A组:盐酸伐昔洛韦胶囊+盐酸多塞平片;B组:盐酸伐昔洛韦胶囊;C组:盐酸伐昔洛韦胶囊+盐酸奈福泮片+盐酸多塞平片;D组:盐酸伐昔洛韦胶囊+盐酸奈福泮片;E组:盐酸伐昔洛韦胶囊+非甾体类抗炎药(包括尼美舒利胶囊、布洛芬胶囊)。统计每组患者性别、年龄、发病至入院时间、治疗后疼痛消失时间,统计分析不同治疗方案患者疼痛消失时间的差异。结果共收集病案资料898份,符合上述5组要求者336例,A、B、C、D、E组分别为72、86、66、60、52例,治疗后疼痛消失时间分别为(5.94±2.54)、(8.60±3.09)、(5.77±1.85)、(5.80±1.96)、(6.86±2.18)d;治疗后疼痛消失时间B组与其余4组差异均有统计学意义(P<0.05);E组与A、C、D组间差异有统计学意义(P<0.05),A组、C组与D组间差异无统计学意义(P>0.05)。结论以上治疗方案均对带状疱疹神经痛治疗有效。盐酸奈福泮片和盐酸多塞平片治疗效果优于尼美舒利胶囊和布洛芬胶囊。盐酸奈福泮片和盐酸多塞平片疗效无差别,且两者联用不增加疗效。
Objective To compare the analgesic effects of different treatments for the treatment of herpes zoster neuralgia and to optimize the treatment of herpes zoster neuralgia. Methods The data of hospitalized patients with shingles were collected from January 1, 2009 to December 20, 2013 according to the military medical record management system. According to the different treatment measures, the data were divided into 5 groups. Group A: valacyclovir hydrochloride capsules and doxepin hydrochloride tablets; group B: valacyclovir hydrochloride capsules; group C: valacyclovir hydrochloride capsules and nefopam hydrochloride tablets and doxepin hydrochloride tablets; group D : Valacyclovir hydrochloride capsules + nefopam hydrochloride tablets; group E: valacyclovir hydrochloride capsules + non-steroidal anti-inflammatory drugs (including nimesulide capsules, ibuprofen capsules). Statistics of each patient’s gender, age, onset to admission time, the disappearance of pain after treatment, statistical analysis of different treatment options for patients with pain disappear time differences. Results A total of 898 medical records were collected and 336 patients met the requirements of the above five groups. The durations of pain disappearance in groups A, B, C, D and E were 72, 86, 66, 60 and 52 respectively (5.94 ± 2.54), (8.60 ± 3.09), (5.77 ± 1.85), (5.80 ± 1.96) and (6.86 ± 2.18) d, respectively. There was significant difference between the two groups in the time of pain disappearance after treatment (P <0.05) (P <0.05). There was no significant difference between group A, group C and group D (P> 0.05). Conclusion The above treatment regimens are effective in the treatment of herpes zoster neuralgia. Nefopam hydrochloride tablets and doxepin hydrochloride tablets treatment better than nimesulide capsules and ibuprofen capsules. Nefopam hydrochloride tablets and doxepin hydrochloride tablets no difference in efficacy, and the combination of the two do not increase the efficacy.
