目的研究姜黄素眼膏对兔实验性小梁切除术后滤过道瘢痕的影响。方法将27只新西兰大白兔按体质量分层随机分为对照组、基质组、姜黄素组,每组9只,均选右眼行标准小梁切除术。术后对照组不做特殊处理;基质组给予约0.1g眼膏基质,3次/d;姜黄素组给予约0.1g姜黄素眼膏,3次/d。分别观察3组眼压、滤过泡、炎性细胞、胶原、成纤维细胞和肌成纤维细胞。结果术前,术后1、3、28d3组眼压值差异不明显(P>0.05),术后7、14、21d3组眼压值有明显差异(P<0.05)。术后7d时姜黄素组与对照组和基质组间的炎性细胞比较差异有统计学意义(P<0.05)。而术后14、28d3组间比较差异无统计学意义(P>0.05)。姜黄素组各个时间点的I型胶原量[7d(21.62±2.87)%,14d(27.83±2.37)%,28d(33.47±2.79)%]与对照组[7d(29.71±1.31)%,14d(36.83±1.73)%,28d(47.13±1.85)%]和基质组[7d(31.07±1.67)%,14d(38.09±1.17)%,28d(48.13±1.69)%]比较差异有统计学意义(P<0.05),但Ⅲ型胶原差异无统计学意义(P>0.05)。姜黄素组的成纤维细胞和肌成纤维细胞与对照组和基质组比较有显著差异(P<0.05)。结论姜黄素眼膏可抑制成纤维细胞和肌成纤维细胞增生,减少Ⅰ型胶原分泌,有效延缓滤过泡瘢痕化进程。
Objective To study the effect of curcumin ointment on the scar of filtration tract after trabeculectomy in rabbits. Methods Twenty-seven New Zealand white rabbits were randomly divided into control group, stroma group and curcumin group according to their body weight. Nine rabbits in each group received standard trabeculectomy. The control group received no special treatment after operation. The stroma group received about 0.1g ointment base once a day for 3 times. The curcumin group received about 0.1g curcumin ointment 3 times daily. Three groups of IOP, filtration bleb, inflammatory cells, collagen, fibroblasts and myofibroblasts were observed. Results There was no significant difference in IOP between preoperative and postoperative 1, 3 and 28 days (P> 0.05). The intraocular pressure was significantly different at 7, 14 and 21 days after operation (P <0.05). At 7 days after operation, inflammatory cells in curcumin group and control group and stroma group were significantly different (P <0.05). There was no significant difference between the 14 and 28d3 groups after operation (P> 0.05). Compared with the control group [7d (29.71 ± 1.31)%, 14d (21.62 ± 2.87)%, 14d (27.83 ± 2.37)%, 28d (33.47 ± 2.79)%] in curcumin group at each time point 36.83 ± 1.73%, 28d (47.13 ± 1.85)%] and matrix group [7d (31.07 ± 1.67)%, 14d (38.09 ± 1.17)% and 28d (48.13 ± 1.69)%, respectively] <0.05), but there was no significant difference in type Ⅲ collagen (P> 0.05). Curcumin group fibroblasts and myofibroblasts compared with the control group and matrix group were significantly different (P <0.05). Conclusion Curcumin ointment can inhibit the proliferation of fibroblasts and myofibroblasts, reduce the secretion of type Ⅰ collagen and effectively delay the scarring process of filtration bleb.
