目的调查桐庐县女性宫颈人乳头状瘤病毒(HPV)感染状况及基因分型特点,为宫颈癌的筛查和防治工作提供理论依据。方法对2014年1月-2015年6月妇科就诊的2 475例女性患者进行HPV基因分型检测,同时分析受检者的相关资料。结果 2 475例女性中有1 011例HPV感染者,感染率为40.85%;不同民族(汉族、畲族)女性HPV的总感染率基本一致,差异均无统计学意义(χ2=1.61,P>0.05)。HPV单一型感染786例,占77.74%,多重型感染225例,占22.26%;汉族与畲族单一型感染率、多重型感染率比较,差异均无统计学意义(χ2=1.71,P>0.05)。高危型HPV检出1 176次(87.70%),低危型HPV检出165次(12.30%),HPV 16、HPV 58、HPV 52、HPV 66、HPV 53位居前5位。15岁~年龄组HPV感染率最高,为72.73%;≥60岁和20岁~年龄组HPV感染率次之,分别为61.90%、61.54%;各年龄段之间的HPV感染率相比较,差异有统计学意义(χ2=66.73,P<0.01)。结论桐庐县女性HPV感染率较高并有明显的年龄分布特点,以高危型单一感染为主;桐庐地区HPV感染分布不存在民族差异。
Objective To investigate the status of HPV infection and genotyping of female cervix in Tonglu County and to provide a theoretical basis for the screening and prevention of cervical cancer. Methods A total of 2475 women with gynecological visit from January 2014 to June 2015 were genotyped by HPV genotyping and the related data were analyzed. Results The prevalence of infection in 1 011 cases of HPV infection in 475 women was 40.85%. The total infection rate of HPV in women of different ethnic groups (Han and Shezu) was basically the same with no significant difference (χ2 = 1.61, P> 0.05 ). There were 786 cases of single genotype HPV infection (77.74%) and 225 cases of multi-type infection (22.26%). There was no significant difference in single genotype infection rate and multiple genotype infection rate between Han and She (χ2 = 1.71, P> 0.05) . High-risk HPVs were detected 1 176 (87.70%), low-risk HPVs 165 (12.30%), and the top 5 were HPV 16, HPV 58, HPV 52, HPV 66 and HPV 53. The HPV infection rate was the highest in 15 ~ age group, which was 72.73%, followed by HPV infection rate in 61% and 61.54%, respectively There was statistical significance (χ2 = 66.73, P <0.01). Conclusion The prevalence of HPV infection in Tonglu County is high and there are obvious age distribution characteristics, which are mainly high-risk single infection. There is no national difference in the distribution of HPV infection in Tonglu area.
