目的:通过对恶性脑膜瘤的CT、MRI诊断加以比较研究探讨其价值及限度。材料与方法:对术后病理证实的20例恶性脑膜瘤术前同时行CT、MRI检查的影像学特征进行回顾性分析。结果:CT像上,病灶内密度不均匀者8例;增强后不均匀强化者4例,病灶不规则边缘呈分叶状和/或结节状者12例,肿瘤包膜显示者2例。MRI示肿瘤呈不均匀信号者,T1WI13例,T2WI14例;增强后呈不均匀强化者14例,肿瘤不规则边缘呈分叶状和/或结节状者14例,包膜不完整者5例,“硬膜尾征”者7例。结论:CT、MRI在显示恶性脑膜瘤征象上是相辅相承的,不可替代的。MRI在显示肿瘤内部纤细结构、轮廓、肿瘤包膜及“硬膜尾征”较CT优越,而对显示肿瘤占位效应、骨质增生、肿瘤边缘及肿瘤强化,CT与MRI作用相当
OBJECTIVE: To compare the value of CT and MRI diagnosis of malignant meningioma with its value and limit. Materials and Methods: The imaging features of 20 cases with malignant meningioma confirmed by pathology before operation were retrospectively analyzed. Results: In CT images, there were 8 cases with uneven density in the lesion, 4 cases with enhanced uneven enhancement, 12 cases with lobulated and / or nodular irregular margins and 2 cases with tumor capsule display. MRI showed tumors were uneven signal, T1WI13 cases, T2WI14 cases; enhanced heterogeneity enhancement in 14 cases, irregular tumor edge lobulated and / or nodular in 14 cases, incomplete capsule in 5 cases , “Dural tail sign” in 7 cases. Conclusion: CT and MRI show the signs of malignant meningioma complement each other, irreplaceable. MRI showed superior stenosis, contour, tumor capsule and “epidural sign” compared with CT in displaying tumor internal organs, which showed the same effect as CT, MRI in displaying tumor mass effect, bone hyperplasia, tumor margin and tumor enhancement
