目的探讨血清中葡萄糖-6-磷酸异构酶(G6PI)和类风湿因子(RF)对类风湿性关节炎的诊断价值。方法采用酶联免疫吸附试验(ELISA)及免疫比浊法分别测定105例RA患者、25例创伤性关节炎、70例健康体检者的血清标本中G6PI、RF的表达水平。结果 G6PI和RF在诊断RA中的敏感度分别为76.2%和83.8%,特异度分别为92.6%和80.0%,两者联合检测敏感度和特异度分别为90.2%和87.5%;105例RA患者血清G6PI、RF含量分别为(5.12±7.93)μg/ml、(25.3±41.2)IU/ml,显著高于创伤性关节炎组(0.12±0.10)μg/ml、(7.5±10.5)IU/ml和健康对照组(0.25±1.12)μg/ml、(2.5±14.1)IU/ml,差异有统计学意义(P<0.01)。结论 G6PI在RA患者血清中高度表达,并且与疾病活动性有密切关系,有望成为RA的新型标志物,其与传统诊断项目RF联合检测可以很大程度地提高RA诊断的敏感度与特异度。
Objective To investigate the diagnostic value of serum glucose-6-phosphate isomerase (G6PI) and rheumatoid factor (RF) on rheumatoid arthritis. Methods The expression of G6PI and RF in 105 RA patients, 25 traumatic arthritis and 70 healthy controls were detected by enzyme-linked immunosorbent assay (ELISA) and immunoturbidimetry respectively. Results The sensitivities of G6PI and RF in the diagnosis of RA were 76.2% and 83.8%, the specificity was 92.6% and 80.0% respectively. The sensitivity and specificity of the combined detection of G6PI and RF were 90.2% and 87.5% respectively. 105 RA patients Serum G6PI and RF contents were (5.12 ± 7.93) μg / ml and (25.3 ± 41.2) IU / ml, respectively, which were significantly higher than those in traumatic arthritis group (0.12 ± 0.10) μg / ml and (0.25 ± 1.12) μg / ml and (2.5 ± 14.1) IU / ml, respectively. The difference was statistically significant (P <0.01). Conclusions G6PI is highly expressed in the serum of patients with RA and is closely related to the disease activity. It is expected to be a new marker of RA. Combined with the traditional diagnostic project RF, G6PI can greatly improve the sensitivity and specificity of RA diagnosis.
