分析20例急性脊柱外伤的磁共振成像(MRI)表现,并对13例脊髓挫伤患者进行了临床随访。20例均在伤后2周内行MRI检查。脊髓挫伤18例,挫伤脊髓段不同程度增粗,呈长T1和长T2信号,向脊髓头尾端延伸2~8个椎骨节段,其中8例有髓内出血,占42%。MRI尚发现15例椎骨骨折,5例硬膜外出血,6例椎前软组织挫伤,6例外伤性椎间盘脱出和3例脊柱韧带撕裂。结果显示,MRI是急性脊髓外伤最佳的影像学检查方法,其尚能很好检出椎体骨折和椎管内外软组织外伤性病变。脊髓挫伤有出血者,临床预后不良。
MRI findings of 20 cases of acute spinal trauma were analyzed. Thirteen patients with spinal cord contusion were followed up. All 20 patients underwent MRI examination within 2 weeks after injury. Spinal cord contusion in 18 cases, contusion of the spinal cord segments varying degrees of thickening, showing long T1 and long T2 signals, extending to the spinal cord head and tail 2 to 8 vertebral segments, of which 8 cases of intramedullary hemorrhage, 42%. MRI also found 15 vertebral fractures, 5 cases of epidural hemorrhage, 6 cases of preoperative soft tissue contusion, 6 cases of traumatic disc herniation and 3 cases of spinal ligament tear. The results show that MRI is the best imaging method for acute spinal cord injury, which can detect vertebral fractures and soft tissue traumatic lesions in the spinal canal well. Bleeding spinal cord contusion, the clinical prognosis is poor.
